Job Purpose/Summary

  • To have responsibility for the provision of specialist palliative care community services to meet the individual needs of patients.
  • To be accountable for defining and ensuring that agreed professional standards are reflected in practice within the community team.
  • To be accountable for the overall management of the community team, recruitment, deployment and supervision of staff and volunteers ensuring the co-ordination of all services provided to the patients.
  • Optimise the cost effective use of all resources.
  • To promote an environment conducive to the training needs of staff and volunteers and participate in teaching.
  • To provide professional leadership.

Accountabilities (Duties & Responsibilities)

Clinical

  • Use advanced clinical assessment for planning, implementing and evaluating programmes of care and prioritising patient care requirements.
  • Manage service caseload and prioritise according to clinical need.
  • Participate in or lead with senior medical staff the weekly MDT meetings, ensuring effective patient management including timely discharge.
  • Act as a resource to the primary health care team providing essential specialist knowledge and advice.
  • In collaboration with the multi-professional/multi-agency teams negotiate care based on needs assessment.
  • Attend outpatient clinics where necessary offering advice and support.
  • Where appropriate use advanced clinical skills to ensure patient care is seamless

Managerial

  • To participate in local NHS palliative care strategy groups and networks to ensure awareness and recognition of the contribution of the activities within the Hospice and to identify future development opportunities.
  • To produce reports, statistics and information as requested and use information to review and develop services.
  • To manage the performance and availability of staff and volunteers throughout the year, including undertaking PPRD’s, managing sickness, absence and annual leave levels, dealing with complaints and participating in the disciplinary process.
  • To recruit and induct staff and volunteers, ensuring that probationary reviews are undertaken and mandatory/core training and education is provided and completed.
  • To participate in research and audit processes as required.
  • To monitor and audit standards to secure and evaluate quality improvements in care.
  • In association with the Lead Nurse and other senior colleagues, to be responsible for project management in specified areas to improve the delivery of care.

Audit and Research

  • Encourage research based practice in relation to palliative/specialist palliative care.
  • Participate in multi-professional audit and research on issues relating to specialist palliative care to underpin practice and clinical effectiveness.
  • As part of the community palliative care team collaborate within the wider multi-professional team to implement nationally agreed standards and audit tools to measure clinical effectiveness.
  • Utilise and disseminate research findings in the delivery of specialist palliative care.
  • Submit statistics as required to meet the requests for activity data from both Community Health Services and National organisations

Education and Training

  • Provide effective teaching to individual patients and their families.
  • In collaboration with the multi-professional teams identify and respond to education needs and requirements relating to the palliative/specialist palliative care.
  • Address the training needs of the wider health community as part of national and local initiatives to enhance palliative care.
  • Act as mentor to both new staff within the community team as required
  • Liaise with external agencies to promote the hospice services and raise the profile of palliative care within the hospice catchment area.
  • Develop service links with other professionals and agencies to optimize the opportunity for patient groups to access our services, i.e. BME groups, individuals who may be socially excluded and hard to reach groups.

Clinical Governance

  • Play a key role in Marie Curie Clinical Governance Programme and local hospice governance groups
  • Participate in, and support research and audit locally, regionally and nationally.
  • Actively encourage and participate in the Clinical Supervision process.
  • Develop research based practice and lead practice development activities.
  • Develop clinical protocols to facilitate practice developments, benchmarking where possible, to ensure that the service adheres to best practice and makes optimum use of resources to achieve the highest standards of care and service.
  • Maintain a safe environment for patients, visitors, staff and volunteers, ensuring that staff and volunteers are aware of their responsibilities in relation to health and safety, and cleanliness.
  • Investigate all accidents, untoward incidents and complaints relating to the service, reporting to the hospice Strategic Management Group and ensure that appropriate action is taken in accordance with agreed policies and procedures.
  • Take an active role in quality assurance initiatives, including the setting and auditing of standards and implementing resulting action plans.

Communication

  • Develop and maintain robust systems to ensure effective communication between patients, carers, nursing staff, allied health professionals, volunteers, the wider multi-professional team, community and hospital teams and other primary care services.
  • In co-operation with the Strategic Management Group and other senior colleagues, utilise and support development of systems to support user feedback.
  • Ensure effective record keeping and documentation.
  • Promote an open and honest culture which reflects a ‘can do’ attitude
