Addiction Medicine Program Fees (effective March 2016)

Dr. Campbell accepts all commercial insurances for the addiction medicine program. Medicaid and Medicare are not accepted. All patients are charged an annual administrative fee which is due at the first appointment of the calendar year. This fee is must be paid by the patient and is not charged to the insurance company.

Co-pays must be paid at the time of service. All balances (co-insurance and deductibles) must be paid within 30 days from the date of service. Patients are responsible to understand what their insurance company will pay and what their responsibility will be. Failure to comply in paying for services will result in termination from Dr. Campbell’s office.

If a cash paying patient has Medicaid, the patient must sign a “waiver of Medicaid rights” annually.

Patients must see Dr. Campbell once every 28 days. Appointments cannot be postponed more than 3 days from the required monthly face to face. If an appointment change is needed you must notify us at least 24 hours prior to the scheduled time. Although Dr. Campbell’s office tries to be flexible excessive changes or no shows will result in termination from the practice.

For patients with prescription insurance Doctor Campbell’s office will submit a prior authorization application when required. The normal time frame is 2 days to 2 weeks, depending on the insurance. Please note that some insurance companies will only allow the authorization for one year. This is the patients responsibility to research their prescription benefits. Patients with limited Rx coverage should discuss a tapering schedule with Doctor Campbell.
