ACADEMIC STAFF (All Academic Staff positions must be reviewed by the Human Resources Department AND by the Academic Staff Title Review Committee PRIOR to the review of the Position Authorization Request (PAR) form by other appropriate authorized officials. A letter verifying the review and the recommended title assignment from both the Human Resources Department and the Academic Staff Title Review Committee shall be attached to the PAR form (ref UWPA 12.08).)

NOTE: A UW-Parkside Recruitment Activity Plan Form must accompany all PAR forms. The Recruitment Activity Plan must include the intent for an international hire, if applicable. For classified positions, this form is only necessary when an exam will be given in order to fill the vacant position. Contact the Human Resources Office if you have any questions.

Instructions for Completing the UW-Parkside Position Authorization (PAR) Form

All new and replacement classified or unclassified positions must receive recruitment authorization approval before recruitment can commence. This requirement is necessary to ensure consistency in establishing title levels, to meet affirmative action guidelines, and to monitor the authorized number of allocated permanent positions for the UW-Parkside campus.

1 / Department / Requesting campus unit (i.e. “Business Services”)
2. / UDDS Code / Seven digit unit/division/department where the position is to be funded (i.e. G02-0200). Call Budget Officer to confirm.
3 / Position Number / Position control number as listed in the campus budget (Blue Book) (i.e. 175.00). Call Budget Officer to confirm.
4. / Salary Range / Two digit salary range code for unclassified positions that coincides with job title (i.e. 05) or three digit salary range code for classified positions that coincides with job title *i.e. 3-06) including the minimum/maximum of the range (ex. $26,712 to $40,071).
5 / Official State Position Title / Must be the title listed in appointment letter
6. / Official State Title Code / (J20FM = unclassified example) or (89102 = classified example)

ROUTING: Prior to routing, ensure that the PAR is complete which includes ALL signatures. This includes the Budget Signature. Once the PAR is complete route to Human Resources at to start the hiring process.
