This packet includes important forms. There are three (3) forms that must be read and retained by you; and two (2) forms that must be read, completed/signed and returned to JCYFA. And there are two (2) documents you’ll need to add (birth certificate and school report) which must be submitted for player certification. 5 / 2012 NMFL Enrollment Affidavit / Return to JCYFA –complete/sign this form and return to your Team Mom on/before the first day of practice. Note, you don’t need a School Representative to sign the form if you are providing a copy of your son’s report card. Your Team Mom will take the player photo during practice.
6 / Player’s Birth Certificate / Return to JCYFA –make a copy of your son’s birth certificate and return to your Team Mom on/before the first day of practice. Note, this document is used by the NMFL to determine player’s age; don’t submit the original, submit a copy onlyas this document is retained by the NMFL.
7 / Player’s School Report Card / Return to JCYFA –make a copy of your son’s report card from school and return to your Team Mom on/before the first day of practice. Note, this document is used by the NMFL to determine player’s school grade; don’t submit the original, submit a copy only as this document is retained by the NMFL. If you don’t have a report card or transcript, there is a section on the “NMFL Enrollment Affidavit” where you can have a School Representative sign to confirm your son’s school grade.

REGISTRATION FEES: Registration Fees are used by the Johns Creek Youth Football Association (“JCYFA”) for team and league expenses including operating & administrative expenses, insurance, use-fees, field maintenance, game uniforms, equipment, coach’s gear, league participation fees, etc. Registration fees also include a uniform allowance for two game jerseys (which you will keep) and ‘rental’ of helmet, shoulder pads and game pants which you will return to JCYFA at the end of the season.
OTHER EXPENSES: Not included in this Registration Fee are other items you will need to purchase separately, including cleats, practice pants/jersey, athletic cup, chin strap and mouth guard. Also, each team usually collects ‘spirit funds’ which your Team Mom will use for awards, team parties, and/or end-of-season banquet/gifts.
EQUIPMENT SECURITY DEPOSIT: A post-dated security deposit check of $150.00 must be submitted to JCYFA at the time of football equipment pick-up; the security check is not cashed unless equipment is not returned in good condition at the end of the season.

NOTE: Additional Equipment Purchase Information - there are several items not included in the registration fee that you’ll need to purchase separately: cleats, white practice pants, white mesh practice jersey, chin strap, mouth guard, and athletic cup. You may already have this gear, but if not, you’ll need to purchase it prior to the first day of practice. Although you can get these items at most sporting goods stores, TITAN TEAM SPORTS is offering JCYFA members a discounted price for all these items.

SAFEKIDS PROGRAM

The following is the policy of JohnsCreek Youth Football Association (JCYFA):

JCYFA is committed to providing a safe environment and to prevent child abuse and sexual misconduct;

JCYFA will make every reasonable effort to ensure that every person involved in coaching/training a sport or activity will abide by the SafeKids guidelines;

JCYFA will make every reasonable effort to exclude any adult with a legally documented history of child abuse/molestation or any other conviction or record that would bring unnecessary risk to the health and safety of the participants of this organization;

JCYFA will perform a criminal background check on every person applying for a coaching or training position at our organization; and,

JCYFA will take appropriate action on all allegations of child abuse and/or sexual misconduct. All allegations will be reported immediately to the authorities for investigation and we will cooperate fully with any such investigation.

To follow is a list of preventive measures which must be followed:

- Physical, mental, and verbal abuses are forbidden

- Inappropriate touching is forbidden

- Coaches/trainers should not socialize with participants outside of JCYFA activities

- Coaches/trainers should not ride solo with a child

- Parents are encouraged to attend JCYFA activities and to transport their child to all activities

- If a child needs special individualized attention, do this with the assistance of another adult

THANK YOU FOR YOUR EFFORTS TO RAISE FUNDS FOR OUR LEAGUE BY PARTICIPATING IN OUR FIRST FUNDRAISING EVENT FOR THE 2012 SEASON!!

This year we have one item to sell so you can recoup the ‘fundraising portion’ of your registration fee.

We will be selling JCYFA raffle tickets. The ticket will be sold for $3 each and we will hold at least two (and possibly three) drawings during the 2012 fall football season (during September and October). The drawing will be for home electronics (computer, laptop, monitor/TV, etc). The great news is that just one raffle ticket qualifies for all drawings. So, a $3 ticket could win a prize valued at $500 or more!!

The funds we raise will help improve our JCYFA equipment & practice fields and will also benefit our sponsor schools including JohnsCreekHigh School, NorthviewHigh School, WilsonCreekElementary School and FindleyOaksElementary School.

Remember, this will also allow you to recoup a portion of your registration fee since you will keep the proceeds of the tickets sales when you sell your tickets – the only thing it costs is a few hours of time.

Here are key points about the tickets:

When you sell the ticket, ask the purchaser to legibly write down their name and phone number on the stub – detach the sections – then they will keep the long ‘ticket’ and you will keep the shorter ‘stub’. Once you have sold all your tickets you must return all stubs to JCYFA by August 15, 2012. The stubs will be placed in a bucket and will be drawn at home games during the season. The winner will be contacted by phone if they are not present at the drawing.

The purchaser’s name & phone number must be written on the stub otherwise there is no way to identify the purchaser.

Each ticket is sold for $3 and will qualify the purchaser for all prize drawings that could be worth over $500 each. All non-winning tickets remain in the bucket for any remaining 2012 drawings. The winning ticket is not returned.

The purchaser does not need to be present to win.

Selling the raffle tickets can be spread out over the next two months since the first drawing will take place in September. However, all stubs must be returned to JCYFA no later than August 15.
