Summary of Fees and Services

I offer several options for patient payment, for your convenience.

  1. Insurance: I am in network with Cigna, Harvard Pilgrim, and Parker Hannifin plans and can bill your insurance on your behalf for these plans. Please read the next page for important details about Insurance Plans.
  1. Wellness Plans: my most convenient, cost effective, and flexible option for patients. This involves a monthly subscription fee for 6 month blocks of time. The first visit should take place in person, but follow ups can be conducted in office or by phone, and additional appointment slots are made available. Please email me at for more information about fees and scheduling.
  1. Out of Pocket per Visit Fees: the following chart shows fees for single office visits. Your insurance plan may reimburse you partial or even full fees if you meet your out-of-network deductible. I am happy to provide coded receipts to you on request so that you can submit them.

Full payment is due at the time that service is rendered. This office accepts: cash, personal checks, Visa and Mastercard credit and debit, including HSA spending accounts.

Most insurance plansconsider me as an “out of network provider.” Please check with your plan manager for more information about reimbursement. Upon request, I am happy to provide patients with a coded form which they may send to their insurance company to request reimbursement.

Prescriptions from pharmacy or natural supplements, if needed, are not included with service fees. Conventional lab orders, pharmacy prescriptions, and some nutritional product prescriptions are routinely covered by insurance policies according to specificpatient plans, even if office visit fees are not, but it is always wise to check with your particular insurance carrier.

For Harvard Pilgrim HMO plans, as well as certain other HP plans, you will need to provide a referral from your PCP in order to have services covered. Please verify this with your plan administrator and fax or send any needed referral forms to me prior to our initial visit. We will not be able to meet for a first visit until I have received this form.Certain United Healthcare plans partnering with Harvard Pilgrimmay be included, but pre-authorization is required for services. Always check first with your plan administrator.

I am unfortunately unable to accept Medicare and Medicaid, as naturopathic doctors are not yet invited to participate in any federal plans. If you have a secondary plan

When lab tests or diagnostic workup is recommended, I can either order these directly, or will attempt to work as part of a team with your primary care doctor to streamline communications, care, and insurance reimbursement for these orders.

I never bill my patients for questions and clarification regardingexisting treatment plans, but new concerns may require a scheduled follow up visit. I strive to ensure that patients are well informed and comfortable with the treatment strategy we have chosen, as well as the conventional medical alternatives so that the best plan of care is matched to each individual.
