Athletic Packet

Parent/Guardian Instructions and Information

ChinookMiddle School offers many sports that are available for seventh and eighth grade students throughout the school year.

Students interested in competing in a sport are required to complete a Sport Packet PRIOR TO PARTICIPATION, which includes:

  1. CMS Athletic Expectations and Requirements
  2. Safety Guidelines Specific to the Sport
  3. Agreement to Obey Instructions Specific to the Sport
  4. Physical Form
  5. Concussion Information
  6. Medical Emergency Release Card
  7. Athletic Fee: $60.00.

HINTS for a successful athletic registration:

Schedule physicals early, as students will not be able to participate without one. Doctors may use either the appropriate section on the yellow Activities Registration Form and Contract or attach a copy of their own physical examination form.

Please make sure to initial the checklist on the outside of the packet as you complete the required pages.

Please make sure that all documents requiring parent/guardian or student signatures are completed in full, accurately and legibly in blue or black ink. Incomplete paperwork will be cause for your student NOT being permitted to practice or compete.

Reminder, all students participating in athletics are required to pay an athletic fee each year of participation. This fee covers all sports for the year and helps offset costs for transportation to and from athletic contests, repair and replacement of equipment, official fees and provides the student with a uniform (jersey) the first year. The fee for a second year of participation is the same as the first year, but without a new uniform being issued.

Students who need to replace shirts may do so for a cost of $20.00 for shirts.

ALL students are encouraged to sign-up for every school sport they even think they might wish to play during the school year at the beginning of the school year. Should the student change their mind and choose not to play a sport later, it is a simple matter to drop them off the computer-generated roster at that time.

The packet will be considered complete only when all of the documents are complete; the parent/guardian, the athletic director or designee initials the packet and when the fees are paid.
