Parish Registration Form

The Office of Religious Educationand Evangelization has begun preparations for 2017 Totus Tuus Summer Youth Parish Mission. We are sending you this Registration Form so that you may reserve a spot for Totus Tuus to come to your parish in the summer of 2017. Please choose three dates out of the dates provided below and number them 1 to 3 according to what is best for you (1 being the best). Note that all parishes do not receive their first choice, so the alternatives must be a viable option. We will send you a confirmation of the date chosen.

Dates will not be reserved until this form, along with a deposit of $800.00 is received by theReligious Education and Evangelization Office. To ensure that you receive the week you choose for Totus Tuus to come to your parish, please return this form and the deposit prior to February 1, 2017. The cost of one team is $1600 with a maximum number of 90 students for the elementary mission and 60 youth for the junior high and high school mission. The cost for two teams is $3000 with a maximum number of 180 students for the elementary mission and 120 for the junior high and high school mission. Two teams require a deposit of $1500.00[*]. The deposit is non-refundable after March 1, 2017. An invoice for the balance will be sent to you in April 2017 with the balance due by May 1, 2017.
