Dean’s Exemplary Student Award – CHS, Glendale Campus

Four CHS-AZ programs will each offer one $500 scholarship (total $2,000) each year.

The programs will offer the scholarship in alphabetical order: during even years, Biomedical Sciences, Cardiovascular Science, Clinical Psychology, and Nurse Anesthesia will offer the scholarships; during odd years, Occupational Therapy, Physical Therapy, Physician Assistant, and Podiatric Medicine will offer the scholarship.

The recipient of the award will be a first year student. The first year class will nominate 3-5 students from their class and forward the names to the faculty; the faculty will select the final awardee from this list of nominees.

The award will be given early-mid May of the first year (possibly at bridging ceremony if program has this type of ceremony).

Criteria for the award could include:

1)professional interaction with faculty and colleagues

2)personal sacrifice to benefit program

3)positive and enthusiastic attitude

4)benevolent

5)collaborative spirit

To process the award for the student:

  1. Programs should reach out to the appropriate financial aid staff member to inquire if the student is able to receive the scholarship as a check OR if the scholarship must be applied to their debt. This is a federal regulation; if a student has reached their cost of attendance, the scholarship must be applied to their debt. If the student has not reached their cost of attendance, they may receive a check. If the student’s last name begins with:
  • A-E: contact Lesa Stanford at 3322,
  • F-K: contact Blanca Ora at 3324,
  • L-P: contact Tammy Schimdt at 3461,
  • Q-Z: contact Stacia McBride at 3246,.
  1. If the student is able to receive a check, then the Program will complete an electronic check request for the check which would be payable to the student, using the appropriate GL account number (the CHS General Scholarship Fund G/L is 31-4090-3105-6470). Business services can provide the fund balance each year.
  1. If the student is not able to receive the check and the scholarship is going to be applied to the student’s debt, then the Program will should provide the following information to the financial aid staff member (as listed above) so they can appropriately apply the scholarship:
  • Name of the student and student ID #
  • Amount of scholarship
  • GL Account number
  • Indicate if the student been told yet so financial aid doesn’t tell the student prior to you announcing if formally or telling the student personally
