This is an exciting opportunity to be involved in the provision of a newly fundedprogram at Star Health. Towards Home – Supported Pathways to Permanent Housing is funded by the Department of Health and Human Services. The Recovery Practitioner initiative provides secure transitional housing to individuals experiencing homelessness matched with on-site support.

We are seeking enthusiastic and experienced practitioners to support the Star Health Towards Home Program in Doveton and Chadstone.The Recovery Practitioner willwork as a Key Worker within a recovery and rehabilitation framework. Recovery Practitioners will support consumers in developing individually targeted goals to increase housing security, increase community connectedness and improve health and wellbeing.

Recovery Practitioners will work across a set roster at one or both of the program sites. Shift hours will be from Monday-Sunday, 7am – 3pm and 2pm – 10pm. Shifts will be negotiated with successful applicants. These shifts may be reviewed dependent on client and service need.

To be suitable for the role you will possess the following:

  • Experience and skills in the delivery of recovery orientated interventions and outcomes to complex consumers
  • Experience in working in Mental Health, AOD, Homelessness or related fields.
  • Strong commitment to the social model of health, recovery orientated practice and trauma informed practice.
