Setup Date/Time: July 14th / 3:00pm - 8:00pm
Location: Shaw Heights Middle School, 8780 Circle Drive, Westminster, CO 80030.
Registration deadline: July 9, 2017. Day of event registration may not be guaranteed.

Thank you for your interest in participating in the annual Hmong American Association of Colorado Summer Festival. This form serves as your registration as a commercial vendor. Please review the logistic to notes below:
- This is a first come first serve registration.
- Registration is accepted the day before the event.
- Non-profits must provide a proof of the 501(c)3 status.
- Space is limited and no electricity and running water will be provided!
- Verbal Commitment will NOT be accepted!

Waiver of Liability
I the undersigned, hereby bind myself, and personal representatives to waive and release Hmong American Association of Colorado, its Officers, Boards, agents, employees or any person or persons under its direction or control from any and all claims or rights to damages for injuries, losses suffered directly or indirectly, including monetary losses, which arises out of voluntary participation as a commercial vendor during the Hmong American Association of Colorado’s (HMAAC) Hmong Cultural and Summer Festival.
I agree to indemnify, hold harmless, defend, release and forever discharge HMAAC, its officers, agents, employees and any person of persons of same, for any liability, claims, damage of any demand, action less or expense (including attorney fees) loss or damage of any kind whatsoever imposed by law upon HMAAC for damages because of bodily injury or monetary losses.
By signing below I have agreed to all of the rules and policies of the Festival regarding commercial vendors and take full responsibilities for myself and my actions. I understand that the HMAAC registration fee is non-refundable and non-transferable for whatever reason.
