Remote Completion of Online Courses Employee Submission Form

Employees of The Evangelical Lutheran Good Samaritan Society may access online training in the Learning Center from locations outside of work with the prior approval of their supervisors. All required courses completed remotely must be documented on this form and submitted to the employee’s supervisor the day after course completion. Courses must be completed in their entirety in order to be considered for approval and payroll processing. Any misuse of remote completion of online courses may result in corrective action.

The hours and times listed above indicate the time that it took me to complete the online course. I understand that I will be paid for courses that are deemed required for my job, for the actual time spent to complete the course as long as the actual time does not exceed the average time listed to complete the course. If my course completion is longer than the average time, I understand I need to contact my supervisor immediately for direction.

Remote Completion of Online Courses Hardware Requirements

Good Samaritan Society employees may access the Learning Center from a non-work location (home, library, coffee shop, etc.) with supervisor approval.

Important Disclaimer: The Society does not support any non-Society off-site hardware (desktop computers, laptop computers, tablet devices or mobile devices) that an employee may use to access the Learning Center. If the Learning Center and/or online courses do not work on the employee’s hardware, the Society is not responsible for troubleshooting or supporting the hardware. In addition, the Society is not responsible for printing issues outside of the Society’s network infrastructure.

In order to access the Learning Center from a non-work location, the computer must meet the minimum criteria identified below. (It is not recommended to access the Learning Center from mobile devices or tablet devices as online courses may not function or report properly.)
