SPRING 2015 APPLICATION

This application is due on Monday, March 2, no later than 3:00 p.m. Late applications will not be accepted!

Directions for submission:

  1. Save your completed application in the following manner only: Lastname_Firstname.doc (example: Nicolet_Jean.doc)
  2. E-mail your completed application to Ms. Reinhart at the following address: <>. Please use your SCHOOL E-MAIL ACCOUNT to avoid your e-mail being rejected by the school’s spam filters.
  3. Submit a hard copy (printed version) of your application directly to Ms. Reinhart in the High School Office (C101). At this time, Ms. Reinhart will confirm that your electronic application has been received.

Please complete all sectionsof this form on a computer (download and then fill out the form using a word processing program; the tables will expand to fit the information as you type). The faculty selection committee will use the information on this form, combined with the teacher survey, to assist with the selection process. Completion of this form does not guarantee selection. It is strongly suggested that you keep a copy of this form for yourself; it will be helpful when completing your college applications.

3.Service Activities: List service activities in which you have participated. These should be any activities inside or outside of school in which you participated for the betterment of your school and/or community.
