ADULT PATIENT REFERRALS TO SECONDARY CARE MENTAL HEALTH SERVICES IN LIVERPOOL

All mentalhealth referrals for adultsin Liverpool should be sent to the Access Team at the Broadoak Unit.

All new patient urgent androutine assessments/referrals

  • 0151 250 5056-24 hour response. Access Team between the hours of 9am to 8pm. After 8pm phone calls for professionals go to the A&E dept at the RLUH
  • 0151 250 5018 (fax)- Urgent assessment within the next few days
  • Send routine referrals viaChoose and Bookto the following service:
  • Adult and Older People's Mental Health - Main Assessment Service Liverpool.

  • 0151 250 5056 Monday to Friday between the hours of 9am to 8pm.

Patients known to mental health services

  • Urgent reassessment-contact your consultant psychiatrist in the numbers below
  • Out of hours only -if the issue is very urgent contact the Access Team up until 8pm.

Things to include when making referrals to adult andolder persons services

  • Current mental health problem, impact on functioning and history/duration of problem
  • Interventions (including medications and psychological) tried and response
  • Current medications
  • Risk history and current concerns about risk/safeguarding issues
  • Contact number of significant carers and other agencies involved
  • Any other relevant issues or comments you think will be helpful.
