ADVANCEMENT TO PROFESSOR, STEP VI, ADVANCEMENT TO ABOVE SCALE/ABOVE SCALE MERIT INCREASE DOCUMENTATION CHECKLIST
Use this checklist when assembling a documentation file for advancement to above scale or above scale merit increase for a candidate in the:

A.Checklist.
B.The original and one copy of the dossier. Additional copies may be requested if they are needed. Each dossier should include:
1.Signed Academic Personnel Action Summary (Form UCI-AP-22).Not required for files processed in AP Review.
2.Dean’s statement.
3.Department chair’s statement (or acknowledgment of review), if provided.
4.Department letter. The department or a designated committee should have reviewed the letter (with notation if copy has been provided to the candidate). The letter should contain a report of the faculty opinion and vote and an evaluation of the candidate’s performance in the following areas:
a.Teaching
b.Research and creative work
c.Professional competence and activity
d.University and public service
5. Candidate’s response to department letter, if submitted.
6. Identification and qualifications of external referees (Form UCI-AP-11).
7.Appropriate number of letters of evaluation, in chronological order, (most recent date first),
(with notation if copy has been provided to the candidate) if provided.
8. Candidate’s comments on redacted copies of confidential letters, if submitted.
9. List of evaluators suggested by the candidate and indication of any considered to be inappropriate, with explanation, if provided.
10. Evidence of Teaching Effectiveness.
- Must include: Student Evaluations of Teaching (individual teaching evaluations and profile of statistical summaries)
- And one or both of the following
- Self-Evaluation Documents (e.g. course syllabi, teaching statement)
- Other Evidence Documents (e.g. peer review of your teaching, awards)
11. Sabbatical leave report, if submitted.
12. Sample letter(s) of solicitation.
13. Addendum (Form UCI-AP-10).
14. Curriculum Vitae.
C.Signed Certification Statement (Form UCI-AP-50).
D.One copy of all publications or creative activity as noted below. Number each publication as it is numbered on the Addendum and Curriculum Vitae.
- For advancement to Professor, Step VI: list teaching and publication record since promotion to full professor.
- For advancement to Professor, Above Scale: list teaching and publication record since advancement to Step VI, or above.
*When needed for clarification, CAP may request teaching data and/or publications from prior reviews.
