6. / BRIEF RESUME OF THE INTENDED WORK
6.1 NEED FOR STUDY:
Perennial allergic rhinitis is a disorder in which there are episodes of nasal congestion, watery nasal discharge and sneezing due to specific reaction to antigens derived from house dust, fungal spores or animal dander. Similar symptoms can be caused by physical or chemical irritants for e.g. pungent odors or fumes, including strong perfumes, cold air and dry atmosphere.
Perennial allergic rhinitis is prevalent worldwide and it is universally acknowledged as an extremely common condition. In day to day practice it forms a major chunk of the outpatient department. In such a common clinical condition the conventional system of medicines advocates anti histamines, topical steroids. If the medical treatment fails surgery is the next option where in reduction of inferior turbinate’s or correction of deviated nasal septum or nose maybe required to improve the airway or to improve access for topical medical treatment. But again the conventional system does not offer a permanent cure and the treatment offered is not bereft of many hazardous side effects. In such a common condition Homoeopathy offers constitutional approach in the management of perennial allergic rhinitis.
Constitutional prescribing is a holistic approach of homeopathic treatment where prescription is not based on isolated symptoms, but on the characteristics unearthed by proper in depth case taking, this information in then matched to a single remedy and this is the ideal homoeopathic treatment.
In the conventional system of medicines, a patient is treated on the basis of disease name, ignoring his constitutional attributes. In Homeopathy the diseased individual with the peculiarities i.e. his constitution is treated, thus paving a way for a holistic, ideal cure.
The present study “ A constitutional approach in the treatment of perennial allergic rhinitis”, is taken up to confirm & unravel the various merits of constitutional treatment ,as there is a pertinent need to study such a common condition of perennial allergic rhinitis and offer solaces to the patient in a rapid, gentle and permanent way.
Null Hypothesis: Constitutional approach in the treatment of perennial allergic rhinitis may be ineffective.
6.2 REVIEW OF LITERATURE:
Definition: Perennial rhinitis may be defined clinically as an inflammatory condition of the nose characterised by nasal obstruction, sneezing, itching, or rhinorrhoea, occurring for an hour or more on most days throughout the year.1
Epidemiology: The disease can commence at any age. Infants who suffer from eczema, often develop rhinitis in childhood, but in these patients, the condition usually improves in early adult life, males and females are equally affected. Causative factors include a hereditary and racial tendency, a specific allergy, hormonal and vasomotor imbalance and psychosomatic effects. More than one of these factors may be present in a particular case.2
Aetiology: Perennial allergic rhinitis may be due to sensitivity to such things as house dust mite, mould spores, animal hair, or feathers and to many other inhaled substances which have been incriminated in various patients. Rarely a food or bacterial allergy is the basic cause of the condition. Hormonal imbalance may cause these symptoms, they are manifested in some patients during pregnancy and the menopause and occur as side effects with some contraceptive pills, and particularly those containing norethisterone .Finally emotional causes are obviously present in some cases where symptoms are initiated, or exacerbated, by stress.
Clinical Features: In perennial allergic rhinitis there are frequent sudden attacks of
sneezing, with profuse watery nasal discharge and nasal obstruction. These attacks last for a few hours and are often accompanied by smarting and watering of the eyes and conjunctiva infection. Skin hypersensitivity may accompany along. 3
Constitutionis defined as a person’s physical and mental makeup whichis revealedthroughhis physicalbuilt,his characteristicdesires, aversions and reactions as well as emotional and intellectual attributes.
  • Physical make-up of the individual illumines many aspects of personality and many of the behavioural patterns.
  • Physical characteristics are linked closely to the genetic set-up and this suggests that the physical and psychic characters are intimately related.
The term constitution emphasizes physical structure as well as the biological function, comprising inherited qualities modified by the environment. The constitution of an individual plays an important role in the drug proving and also in the occurrence of natural diseases. Hence the role of constitution is crucial in treatment and achieving a cure. The Knowledge of constitution of a medicine and constitution of a person on the part of a physician helps to arrive at a greater level of similarity between the person and the medicine.4
To understand the constitution of a person, a physician must know the person’s physical make up, namely, his build, desires, aversions, and disagreement with foods, characteristics of his discharges and eliminations, and his thermal reactions. Also mental attributes, namely, emotional and intellectual reactions and performances. Various concomitants expressed in relation to the stressful situations like dentition in a child, menstruation, pregnancy and menopause in a female are important expressions which reveal the quality of a constitution.5
Mathur KN quotes Kent who was of the opinion that it is the sick individual as a whole to be cured rather than the totality of characteristic symptoms
or the pathological condition of the diseased organ of the patient. Therefore, he gave highest or first rank to the general characteristic symptoms of the patient as a whole. These symptoms are also called the constitutional symptoms of the patient.6
Boericke opines that ‘Constitutional’, classical, totality, genuine or Hanemannian homeopathy, these titles all mean essentially the same thing, they denote a precise definition of homeopathy in which there must be a proper in depth case taking and understanding of the patient and this information must then be matched to one single remedy. Any other way of using potentised remedies isn’t actually homeopathy but something adapted from it and so, being different, it won’t operate according to the same paradigms and so could be superficial or even questionable.7
Banerjea puts forth the miasmatic cleavage of perinneal allergic rhinitis as follows:
Psora : Psoric hay fever is charecterised by sneezing, redness and heat and by sensitiveness and a watery discharge .
Sycosis : In Sycotic hay fever the discharge is scanty but the patient cannot breath through the nose or blow any mucous from it. The slightest discharge however relieves the stopped up feeling.
Syphilis : Syphylitic discharges from hay fever are acrid, putrid and offensive .
Tubercular :Tuberculkar hay fever is periodic and recurrent with much sneezing and various allergic manifestations . the nose may clear one hour and be extremely stuffed-up the next. These cases are more difficult to cure and often characterised by thick and occasionally blood streaked discharges.8
Hughes quotes ,Dr.Jousset who says that hay fever as a manifestation under the influence of a special irritant ,of what he calls “gouty coryza ”- an affection characterised by paroxysm of nasal influx and repeated sneezing ,which he has never seen save in gouty and haemorrhoidal subjects. The remedies that he
has recommended are namely Kali chlor, Ars alb, Ars iod ,Nux vom,Camphor,Iodine, Pulsatilla,hydrastis.9
Kent in his repertory gives the following remedies under the sub rubric annual ( hay fever ) :10
1ST Grade Remedies: All -c, Arundo,Nat.mur ,Psorinum,Sabad,Sin-n.
2nd Grade Remedies: Ars,Ars iod,Arum- t,Brom,Carb v,Dulc,Euphr,Gels,Kali –i,kali-p,Naja,Puls, Sang,Ran-b,Sil,Stict,wye .
6.3 OBJECTIVE OF THE STUDY:
  • To study the clinical presentation (individualistic) of Perennial allergic rhinitis.
  • To study the constitutional approach in the treatment of Perennial allergic rhinitis.
  • To study the miasmatic cleavage of Perennial allergic rhinitis.
  • To compile the data of Perennial allergic rhinitis and constitutional approach from various sources in homoeopathic literature
7. MATERIALS AND METHODS:
7.1 Source of data:
Patients will be selected on the basis of inclusion & exclusion criteria from Bharatesh Homoeopathic Medical College & Hospital’s, OPD/IPD and Village Health Camps.
7.2 Method of collection of data:
Patients will be selected on the basis of inclusion & exclusion criteria.Minimum sample size will be 30. Simple random sampling procedure will be adopted. All cases will be taken as per the proforma prepared for the study. All cases registered between 12th Dec, 2012 to 15th March, 2015 will be included for the study. No new cases will be registered after 12th Dec, 2013 but however follow up of the cases will be continued till 12th Dec, 2014. The follow-ups will be taken every weekly, fortnightly, monthly or as per the requirement of the case.
Inclusion criteria:
  • Patients of all age group, both sexes irrespective of the socio economic status will be taken for the study.
  • Patients confirming to diagnostic criteria of Perennial allergic rhinitis.
Exclusion criteria:
  • Cases with gross pathological changes and complications.
  • Patients on immune suppressants and on any other active treatment.
Research hypothesis:
Constitutional treatment is effective in the treatment of perennial allergic rhinitis.
Null hypothesis:
Constitutional treatment is ineffective in the treatment of perennial allergic rhinitis.
7.3 Does the study require any investigations to be conducted on patients, or other humans (or animals)? If so please describe briefly.
The diagnosis of the case will be done on the basis of case history and clinical findings, therefore no specific investigation will be done. However absolute eosinophil count and serum IgE will be advised depending upon the merit of the case.
7.4 Has ethical clearance been obtained from your institution in case of 7.3?
