  Extra Medical Release Forms

  Permission slips for tobacco use

  Money Bag or Envelope & Money Form (to collect and record incoming money)

What to Get from Students:

  Final balance (amount varies, depending on any fundraisers they turned in)

  Medical Release Form 2.0 – signed by parent AND student

  Copy of Medical Insurance Card – front & back.

  Their Picture (take each student’s picture as part of the registration).

What to Give to Students:

  Camp info letters

  Release Forms, as needed

Registration:

  An orderly process should be set up to include the following:

1.  Collect Medical Release Form, note on Money form

2.  Copy front & back of Medical Insurance Card, note on Money form

3.  Collect money and record it on Money Form (be sure to indicate students who pay cash)

4.  Make sure they have a Camp Info letter for the meeting

5.  Direct student to get their picture taken

  Late: Make notes to spreadsheet indicating amount of money turned in that night, as well as forms/copy of medical insurance card collected

What to Give to Jim after Parent Meeting:

  Money Form filled out and money


What to Cover:

  Show Promo Camp video as students are registering

  Intro staff – Bethany, Josh, Kim (not here tonight: Jodie, Sara, Fabian, Monty)

  Indicate what you need from students:

o  Final balance – if you cannot turn it in tonight at meeting talk to Tim or Marcy after meeting.

o  Medical release form version 2.0

o  Copy of the front & back of medical insurance card. If you don’t have insurance, you will be covered under YFC insurance.

  Go over Camp Information Letter, highlighting key info:

o  Date and Times

§  Eat before you come on Sunday, July 19th.

§  Schedule: Every day, students will participate in a session with a speaker and band, as well as small group discussions.

o  Different “rides” (i.e., horseback riding, high ropes course, climbing wall) will be scheduled each day, as well as “surprise” activities throughout the week.

o  Plenty of free time to enjoy the other activities that are available at camp, such as frisbee golf, mountain biking, lake, canoeing, zip line, the Blob & a water slide, game room, indoor gymnasium, etc. There are no extra costs for any of these activities.

§  Do not bring electronic items.

§  Health Concerns: There will be a nurse, physician, or EMT at camp at all times to deal with any illnesses or injuries.

§  Students will be responsible for their own medications while traveling. However, all prescription medications AND over-the-counter drugs MUST be turned in upon arrival at camp and will be administered through the nurse’s station all week. Please make sure medications are labeled with your name and “Grand Rapids YFC”, and packed in your carry-on.
