The latest release to FormsNet™ 2 (FN2) was completed on January 4, 2010. Although the release focused on the creation of a Clinical Trials module, the following changes to the Recipient module were included:

Security Change:

If a user attempts to log into FormsNet™ 2 unsuccessfully five times in a row, that user will be ‘locked out’ of logging in again for 10 minutes. After the 10 minutes, the user can try again. This security feature cannot be overridden by the HelpDesk and there is no tool within FN2 to override this. The only course of action after five consecutive failed logins is to wait 10 minutes and then try again. This change is required to comply with United States federal regulations (21 CFR part 11).

Features Fixed and/or Functionality Repaired:

Form 2004, 2005 and 2006: Earliest Complete Date and Due Date will now be correctly calculated for different instances.

Form 2005: Validation has been corrected to allow a single HLA allele typing to be filled either at the first or the second position.

Form 2503/2504: KGF forms will automatically be made due when appropriate questions are answered on the 2400. The PDF of the KGF Study form is now available.

Form 2802: Users can no longer create the form 2802 and should submit a paper version of the form.

Center Forms Due: date filters are working properly and generate an accurate report.

Recipient forms grid: IUBMID and NMDP Recipient ID columns have been added.

Forms due rules:

·  have been corrected on the Pre-TED so appropriate number of 2004, 2005, and 2006 forms come due

·  pertaining to disease specific forms have been reviewed and corrected so that appropriate forms are made due

·  have been corrected so that if a Form 2400 is reprocessed and a Form 2900 has been completed, then additional forms will not be made due
