CCISD TAKE HOME MAKERSPACE KIT PERMISSION SLIP

Please return this signed form to your school librarian.

Your student’s school library allows students to check out and bring home a makerspace kit. These kits provide tactile materials for your student to create and learn off campus. This allows students who are unable to visit the library and work on creating, tinkering, coding, and learning to do so at home. Students are expected to return the kits to the library with most of the pieces intact that are indicated below. If the kits are returned missing items, your student will be charged the cost of those missing items.

If you allow your student to participate, please sign this permission slip and indicate which of the kits you are allowing your student to check out.
