ATTENDANCE SEIT (white) CARDS INSTRUCTIONS
1. All Entries must be made by the SEIT in blue or black ink. Changes should be made in red ink by drawing a line through the original entry, and recording new data. Erasures are prohibited as attendance cards may serve as evidence in court.
2. Data to be recorded on the Attendance Card.
a. Student’s name - Enter last name and first name.
b. Current New Class – Class designation used on the school organization chart is entered in first box under heading “Fall”.
c. Parent/Guardian – Enter full name of parent or guardian. If the student resides with a person other than a parent, enter name and relationship in addition to parent’s name.
d. Address – Enter complete address including apartment number or location, and zip code. Verify addresses periodically. If student moves enter new address on second line and submit a T-2P form to CPSE/CSE.
e. Date of Admission – Enter the date the student was admitted into your program for the current school year. For two-month cards the earliest date would be July 1st of the current school year.
f. Date of Discharge/Prom. – Enter date of discharge file card in the back the student’s school folder.
3. Attendance Form D:
All information reported on the student’s attendance card must be accurately transcribed onto the Attendance for D that is submitted to the Bureau of Contract Aide.

School Information
a. School Name – Enter the name of the school: Important Steps, Inc. (as it appears on the SED approval letter).
b. School Code # - Enter the letter “C” for preschool and the three digit code (C 169) assigned to the site by the New York City Board of Education.
c. Teachers Name – Enter the name of the SEIT providing the CPSE Special Education Itinerant Services.
d. Class – N/A
Student Information
Enter information as requested.
Daily Attendance- 39 Weeks (42 Regular Weeks Minus 3 Weeks for Holidays/Recess/Etc.): Sept. 8, 2009 – June 24, 2010
Place a mark in each box, as appropriate:
· “X”- Present
· “E”- Legally absent (see list below)
1) child’s illness and/or hospitalization (Dr’s note for more than 1 week of absence)
2) death in the immediate family (no more than 5 days per occurrence)
3) family member illness, emergency (note from parent is required for more than 2 days)
5) religious observance
6) transportation issues bringing the child to school
7) unusually severe weather
8) other reasons approved by NYS Commissioner (provided upon specific notification)
· “A”- Illegal absence the day the session was scheduled
· “C”- School or Important Steps, Inc. are closed for conference/staff development.
· “P”- Provider absent
· “R” - Official school recess periods: Winter, Mid-Winter, Spring (must correlate with Important Steps, Inc. calendar
Provide explanation of all “E”legally absent and “A” illegally absent missed sessions on the back of the card (Notify Important Steps, Inc. immediately if more than 5 consecutive scheduled sessions missed) Session notes must correspond with all marked dates.
Admission/Discharge
a. Date Admitted - All two month (summer) cards must indicate the student’s first attend date in July or August or the date eligible to attend but legally absent.
b. Date Discharged - Enter the date of discharge only if the student is discharged from the program prior to the last day of the program. Do not enter a discharge date for students who remain in attendance at the program through the last day of class.
c. Reason - Provide an explanation, as appropriate, for the discharge.
Daily attendance cards must be signed by each provider (SEIT) and dated at the time service is terminated or when the service period ends. Cards must be retained by the school in accordance with applicable contractual obligations.
