PPP Closeout Report Template

The template presents an outline for a PPP final report. The template is complete with an approval form to be completed upon review of the PPP final report at program closeout. The implementing partner is responsible for completing the closeout procedures. The template assumes formal communication on PPP closeout has been disseminated to all PPP partners.

Approval of the PPP Closeout Report indicates an understanding and formal agreement that the project is ready to be closed. By signing this deliverable, each PPP partner agrees all administrative, financial, and l deliverable aspects of the project has been concluded

Executive Summary :Identify the PPP key activities, deliverable and results and indicate key partners who were involved in program implementation. Highlight the final schedule duration of PPP activities and report on the activities which would need to be sustained by the public partner through other contractual mechanisms. Highlight total project expenditures and include variances from original budget estimates. Report on the final total in-kind and monetary contributions from both the public and the private partners.
