Remarks:

1. / Priority will be given to the primary schools which have participated in the Easy Sport Programme - Basketball under the SSP between 1/9/2016 and 31/12/2017. All participants should be current students of their schools during the Competition. If the quota is not filled, other school teams are welcome to join the Competition.
2. / Priority will be given to schools which have participated in the basketball course under the Easy Sport Programme between 1/9/2016 and 31/12/2017. Places will be allocated by ballot according to the priority shown on the enrolment form in the maximum of 2 teams for each school (i.e. Team 1 & 2). Team 1 of each school will be allocated first and Team 2 will be allocated after all Team 1 enrolled. If remaining quotas still exist, places will be allocated to other schools which have not participated in the basketball course under the Easy Sport Programme by ballot.
3. / A separate form is required for each team.
4. / Each participant may play for one team only. Any change of the participant list is not allowed once the payment is settled.

6. / The information provided by you will only be used for the enrolment of recreation and sports activities organised by the Organiser and the Leisure and Cultural Services Department, future contact purpose, compilation of statistics and opinion survey. Only those who are authorised by this Department will have the access to such information for the aforesaid purposes. For correction of or access to personal data by means of this forms, please contact the staff at 2601 8861 or 2601 7602.
If no “Acknowledgement Receipt” is received from the LCSD two days before the place ballot date, please contact our staff at 2601 7602.
7. / The applicant should fill in the personal information in the application form. If you do not provide the required personal data, the Leisure and Cultural Services Department and the Organiser may not be able to process your application.

All participants already have the consent of their parents/guardians or the persons authorised by their parents/guardians to participate the above activity, and they do not suffer from any illness that renders them unfit for the activity. The Organiser and the Leisure and Cultural Services Department shall not be liable for any injury or death which the participant may suffer in this activity, if the cause of injury or death is due to his/her negligence or inadequacy in health and fitness. / School Chop：
