Parental Consent Form
Dear Parent:
Baylor College of Medicine (BCM) is pleased to offer your child an opportunity to participate in our Saturday Morning Science program. The program is designed to broaden the life experiences of students and help prepare them for careers in medicine, science and the health professions. It is headed by Dr. James L. Phillips, Senior Associate Dean and Professor of Pediatrics in the Office of Diversity and Community Outreach at BCM.
We plan to offer many experiences to the SMS students, which will help them grow personally as well as intellectually. The SMS students will attend lectures and workshops intended to peak their interest in the biosciences—some of which include cell biology, anatomy, and histology. We will end the program with a college fair that will feature area colleges and universities.
We enlist your support in our efforts to host a program that is second to none. Please carefully review the student contract (below) with your child and return it to us along with a transcript, a signed photo release form, and this consent form. We ask that parents and other guests NOT ATTEND any of the lectures, unless otherwise specified. We ask that you expedite the student’s travel so that he/she arrives at 8:30 am and is picked up by 11:30 am. All lectures start at 9:00 am and end at 11:30 am except for special sessions. A schedule and directions will be forwarded to you if your child is accepted for enrollment into the program.
If for any reason your child is unable to attend a session, please contact Laura Perry at 713.798.8646 or to let her know in advance. After three unexcused absences, the child will be dropped from the program. Please return these forms to the Saturday Morning Science program by Monday, November 30, 2015:

Saturday Morning Science Student Contract
As a student of the Saturday Morning Science (SMS) program at Baylor College of Medicine (BCM), I ______(your name) agree to the program guidelines listed below.
I agree to:
- Attend all sessions of the SMS Program unless my absence is excused. I understand that an excused absence is an absence that my parent or guardian notified the program coordinator of in advance, or in cases of illness. I understand that after 3 unexcused absences, I will be dismissed from the SMS program.
- Arrive on the campus of BCMbetween8:30am and 8:45 am on program days.
- Wear business casual attire on all program days unless otherwise instructed.
- Honor the BCM policy for an alcohol, drug and harassment free environment. I understand that if I violate this policy, BCM reserves the right to terminate me from all program activities.
- Conductmyself in a professional manner. My cell phone will be turned off during SMS sessions and activities. I will not loiter outside the building. I will show respect for myself and others while on the campus of BCM.
- I will respect all BCM/SMS equipment entrusted to me, facilities, staff members, and volunteers.
