On the Job Injury - Employee Instructions
If you are injured on the job - See the school nurse. She will do a preliminary assessment of your injury and treat if possible. The nurse will also have you complete a DWC1 - First Report of Injury.
Any employee who is injured on the job must have a completed form DWC1. The form is already pre-filled with the District’s name and address AND even more importantly, our Workers’ Comp Insurance company’s information. The hospital must have both bits of information in order to cover under Workers’ comp.
The nurse will make a copy for the injured person before they leave for the hospital (unless of course it is a life or death emergency). The original should be sent to the Superintendent's Secretary, Angela Fudge.
Bits of Information:
Ø The hospital emergency room is the only facility in town that accepts Workers’ Comp patients. Yes. That is correct. There are not any doctors in Crockett or Houston County that will treat a patient under Workers’ Comp. However, there are doctors who will treat Workers’ Comp patients in Palestine, Lufkin, Tyler & Nacogdoches.
Ø There are 3 pharmacies that will fill prescriptions under Workers’ Comp in Crockett: Wal-mart, Brookshire Bros, & Davy Crockett Drug.
Ø Please keep in mind that the employee will be billed for all charges if the hospital, clinic or pharmacy does not have the proper Workers’ Comp info. The result:
o Employee receives bills & invoices.
o Employee knows it was a Workers’ Comp Injury so does not pay.
o Bills get turned over to collection agency.
o Black mark on employee’s credit.
If the injury occurs during hours when a school nurse is not available, the injured party may complete the DWC1 found on our website.
Be sure and bring a copy of the DWC1 AND a copy of the reports from the hospital to Angela Fudge. Ask the hospital for copies if they do not automatically give you one.
