Applications are invited to apply for a locum Consultant post in General Paediatrics based at the RoyalHospital for Sick Children, Glasgow that forms part of the Women and Children’s Directorate. This post is for a period of 12 months

RHSC, Glasgow, is the largest paediatric teaching hospital in Scotland and is a world leader in the provision of child and adolescent health services. It provides care, not only for children resident within Greater Glasgow, but is also a tertiary referral centre for children from the West of Scotland and, in some sub-specialties, from the whole of Scotland and wider.

The Directorate is committed to supporting the development of a Consultant- delivered service. On call responsibilities are for acute General Paediatrics.

Applicants must possess full GMC registration and a licence to practise.

SECTION 1: Children’s Services across NHS Greater Glasgow and Clyde

The Yorkhill campus is the site for the RoyalHospital for Sick Children and limited on site laboratory facilities. Children’s services are delivered on this and a number of other sites across the Health Board.

The RoyalHospital for Sick Children

RHSC is one of the largest paediatric teaching hospitals in the UK and is the largest in Scotland. It provides paediatric secondary care for the 900,000people resident within the Greater Glasgow and Clyde area and tertiary care for the three million residents across the West of Scotland. There are a number of National designated services delivered from the hospital including cardiac surgery, renal transplantation, bone marrow transplantation, extra corporeal life support and complex airway services.

There is a complement of 211 beds, including 23 integrated neonatal medical and surgical cots. There are 16 nationally funded paediatric intensive care beds and 6 high dependency beds within an integrated critical care facility. A new theatre complex was opened in 1998, comprising 7 full theatres, a scope facility and dental suite. The emergency department sees in excess of 45,000 patients per year.

All paediatric medical and surgical specialties are represented within the hospital. These include general medical and surgical paediatrics, neonatal surgery, burns, plastic surgery, ophthalmology, and dental surgery, orthopaedics, ENT, cardiology, neonatology, neurology and neuro-surgery, respiratory, nephrology, gastroenterology, rheumatology, dermatology, immunology and infectious diseases, haematology and oncology, metabolic medicine, diabetes and endocrinology. There is a dedicated Child Protection Unit. Child and adolescent psychiatric services are located within the campus, along with a clinical psychology department.

There is an on site radiology department, which provides ultrasound, CT, MRI and isotopic studies. The department is film-less and accessed electronically. There are some diagnostic laboratory facilities on site including haematology, blood bank, biochemistry and genetics.

The hospital has close ties with the University of Glasgow and is the major undergraduate paediatric teaching facility. It accommodates the University Departments of Child Health, Child and Family Psychiatry, Medical Genetics, Human Nutrition, and Paediatric Surgery. The department of clinical audit and research is available to assist and support research projects.

Other paediatric services across NHSGG&C.

The RoyalAlexandriaHospital, Paisley, provides both in patient and out patients care for paediatric patients across Clyde. This unit also houses a neonatal intensive care facility.

There are two maternity units within Greater Glasgow, the RoyalMaternityHospital and the Southern General Hospital. Both have extensive neonatal intensive care facilities.

Specialist community based children’s services can be found through greater Glasgow and Clyde. These services are managed within community health and social care partnerships but have close links with the Yorkhill campus.

New Children’s Hospital

A new children’s hospital has been commissioned on the site of the existing Southern General Hospital. This is running concurrently with the commissioning of a new adjacent adult hospital. This project is to assist the development of improved child, maternal and adult services on one combined site. This would make the Southern General one of the largest campuses in Europe. The anticipated opening date is 2015.

Clinical Leadership

Medical services are a key component of integrated services within the Acute Operating Division, Women and Children’s Directorate, NHSGG&C.

Dr Jim Beattie is the Associate Medical Director for Women and Children’s services.

Future Service Developments

The Scottish Government launched the National Delivery Plan for Specialist Children’s Services in the period 2008-2011. Resources were invested in a number of specialties including critical care, interventional cardiology, childhood cancer, complex respiratory and airway services, metabolic medicine, immunology, cleft surgery and childhood diabetes. These services continue to develop.

A surgical service for Hypoplastic Left Heart has been developed.

Neurosurgical services have recently moved on site and are co-located with paediatric neurology. This development will continue to improve the multi-disciplinary care required for this complex patient group.

There are now formalised speciality networks working across the West of Scotland, Lanarkshire and ForthValley to help support locality care. These include metabolic medicine, gastroenterology, diabetes and endocrinology and general surgery.

SECTION 3: Post

1. Title

Locum Consultant in General Paediatrics

The successful incumbent will contribute to setting up an integrated and dedicated general paediatric department. Successful candidates will work closely with colleagues in general paediatrics and emergency medicine.

Acute medical admissions are initially assessed and managed in the Emergency department and adjacent Medical Assessment Unit (MAU). The MAU has 12 beds plus a bed space for the observation of adolescent self- harm. This unit assesses approximately 10,000 medical patients each year with an admission rate of one third for continuing in-patient care. There is marked seasonal variation in activity.

While in the MAU, a patient is under the care of the ED consultant or general paediatrician assigned to cover MAU. Once the decision to admit has been made, the patient is under the care of the acute medical receiving consultant.

Acute medical paediatric receiving is currently undertaken by a team of 11 consultants. There are two dedicated general posts and the rest of the team have other specialty commitments when not receiving. During receiving weeks the on-call consultant cancels all other clinical commitments. The attending consultant is responsible for the day to day care of all acute medical paediatric patients during their 7-day on call period. This involves attending the morning hand over with overnight staff and two daily ward rounds. Patient admitted to PICU/HDU during this period are also the responsibility of the medical consultant, unless known to another specialty.

Out of hours cover is expected on 4 nights out of the 7, with a second consultant providing the out of hours on call on the alternate 3 nights.

The receiving medical consultant is also responsible for cases of physical child abuse, which present after normal working hours and may be required to assess such patients during the day time if there is no other MAU cover.

The above is the current acute receiving process but this will be undergoing re-design to provide more extended senior cover through out the day and to make provision for the much busier winter period. Significant changes in work pattern are anticipated across the women and Children’s directorate.

A further change is likely to be the development of an integrated acute medical and surgical unit, which will be in line with the plans for the new children’s hospital.

3. Duties of the Post

Participate in the general receiving service the RoyalHospital for Sick Children.

Contribute to consultant delivered care in greater Glasgow and Clyde.

Conduct general out patient clinics as per job plan in hospital and in community settings.

Liaise with other agencies within the hospital and community setting in the promotion of seamless children's services.

Contribute to the planning and development of the General Paediatric Services in Greater Glasgow and Clyde, including information collation, evaluation of services, service and policy development.

To work with the Clinical Director and Lead clinicians in the further development of integrated care pathways for general paediatric care across Greater Glasgow and Clydeand provision of acute care.

Contribute to undergraduate and postgraduate training and supervision and mentorship of junior doctors and other health professional as appropriate.

Undertake management responsibilities within general paediatrics.

Participate in audit and research activities within the service.

4. Provisional Job Plan

These will be 10 session posts with a 9 and 1 split. Individual commitments may be subject to alteration during the absence of colleagues. Duties at other hospitals may be undertaken as necessary.

Fixed commitments (Non-receiving weeks)

Successful candidate will be expected to perform 3 general out-patient clinics weekly. These may be located in the main RHSC OPD or in out reach localities.

3 timetabled sessions to provide medical cover for MAU. This will include responsibility for any child protection cases which present during the period of cover.

2 timetabled sessions for clinically related activities.

1 SPA

Involvement in electronic vetting

Supervision and training of undergraduate students and paediatric trainees.

The successful candidate will also be encouraged to participate in management, service development, clinical audit and research.

Receiving weeks and weekends are as scheduled by rota.

5. Review of Job Plan

A formal job plan will be agreed between the appointee and the Clinical Director on behalf of the Associate Medical Director before commencement.

A job plan based on 10 programmed activities per week contract would include:

1 PA allocated to emergency care out-of-hours and weekends

8 PA’s allocated to direct clinical activity

1 PA allocated to supporting professional activities

Work will be arranged with ‘hot’ weeks of clinical receiving work in the receiving wards. Outpatient and non-clinical activities are concentrated in other weeks. The opportunity to undertake additional clinical sessions may be available if mutually agreed. Following appointment the successful candidate can discuss varying the SPA commitment provided additional specific activities can be mutually agreed with the lead clinician or Clinical Director.

The Job Plan will then be reviewed annually, following the Appraisal Meeting. The Job Plan will be a prospective agreement that sets out a Consultant’s duties, responsibilities and objectives for the coming year. It should cover all aspects of a Consultant’s professional practice including clinical work, teaching, research, education and managerial responsibilities. It should include personal objectives with details of these might link to wider service objectives, and suggestions on how the consultant can be supported to fulfil their stated objectives.

Accreditation requirements from prospective applicants

Applicant should have completed Specialist Training or equivalent and be entered on the GMC Specialist register or be eligible to be listed within six months of interview.

Office Accommodation/ Secretarial support

The successful applicant will be provided with office accommodation including IT facilities. Secretarial support will also be provided but it is likely that this will be shared with colleagues.

Maintaining Medical Excellence

The organisation is committed to providing safe and effective care for patients. To ensure this there is an agreed procedure for medical staff that enables them to report quickly and confidentially, concerns about the conduct, performance or health of medical colleagues (Chief Medical Officer, December 1996). All medical staff practicing within the organisation should ensure that they are familiar with the procedure and apply it.

The Health Board has a Risk Management Strategy. All Consultant Medical Staff are required to adhere to the principles and practices contained therein.

6. Other Activities

Teaching

The Health Board has a commitment to the teaching of undergraduate students from the University of Glasgow. The successful applicant would be expected to participate in both informal and formal teaching programmes for undergraduates and all levels of junior medical, nursing, and paramedical staff where appropriate.

Research

The department is actively involved in research and is affiliated to the University of Glasgow. The lead clinician for Research and Development for Children’s Services is Professor Faisal Ahmed.

Clinical Audit and Clinical Governance

The successful candidate must be aware of clinical governance and clinical risk management and take an active part in their implementation, including audit. Clinical audit in the hospital is supported by an Audit Department with appropriate staff.

Continuous Professional Development

Continuous professional development is supported according to the guidance of the Royal College of Paediatrics and Child Health.

The successful candidate will become members of the Yorkhill Medical Staff Association and the Division of Child Health.

Leave

1 WTE is granted 6 weeks annual leave and 10 days per year of statutory public holidays. Consultants in the department participate in cover for annual and study leave. Absence must be planned in advance by discussion with consultant colleagues.

General Provisions

You will be expected to work with local managers and professional colleagues in the efficient running of services and will share with Consultant colleagues in the medical contribution to management. Subject to the provision of the Terms and Conditions, you are expected to observe the Division’s agreed policies and procedures, drawn up in consultation with the profession on clinical matters, and to follow the standing orders and financial instruction of NHS Greater Glasgow and Clyde. In particular, where you manage employees of the Division, you will be expected to follow the local and national employment and personnel policies and procedures. You will be expected to make sure that there are adequate arrangements for hospital staff involved in the care of your patients to be able to contact you when necessary.
