PARENT APPROVAL FOR ROCK CREEK FIELD TRIP
I give my permission for ______to take a field trip to Rock Creek Park for water quality testing. We will have goggles and latex gloves on for our own safety. The date and time of the field trip will depend on which period your son/daughter has science. Check the chart at the bottom for this information.
Cost will be $2.00 per student and will need to be brought to the office at least a day before their scheduled field trip date. The permission slip needs to be returned to their teacher at least the day before they are scheduled to go to Rock Creek. The Western States Bus Company will provide transportation.
Since this is an outside field trip, please do not wear your best clothing. We will go on this field trip regardless of weather, so please come prepared for the day’s weather condition!
I further agree to assume the responsibility of seeing that my child follows the directions and instructions of the school official(s) in charge. Careful planning has gone into the field trip and reasonable safety precautions have been taken; however, O’Leary JHS is not responsible for injuries while on this school trip.
