SCWDC Activity Leader Application Form

The Activity Leader Evaluation Committee (ALEC) is preparing to select next season’s trip leaders. If you wish to be considered for a trip leader position, complete and submit this form, which must be received at the SCWDC office, or a member of ALEC by April 16, 2016. Email = Bernie Gilbert, .

• Missing this deadline places you at the very bottom of the leader list for this year.

•Include all services you are scheduled to provide through the end of the club year (30 April).

•Print or type or print clearly in black or blue ink. If your writing is not legible, you may not be awarded points for your activities.

•Complete all applicable sections. Be as specific as possible. If you can not find an exact match for a position, please describe your position on an attachment. If you run out of space, attach the additional information. Remember: nearly everything you do for the club can earn you points. If you don’t write it down, you don’t get the points!

•Do NOT complete the Grey sections; they are for use by ALEC.

•ALEC will evaluate the activities you list on this form, and will award points based on the SCWDC Point System Guidelines. (The Point System Guidelines are specified in Appendix 13 of the SCWDC Policy Manual.)

You will receive in the mail, a copy of ALEC-evaluated form, which you should keep for your records.

IV – Leadership and Volunteering
Trip or activity leaders, assistants, and volunteers should complete this section.
A leader is the individual responsible for the execution of an activity, is responsible for the Activity Financial Report (Yellow), and is usually compensated with free admission to the activity or trip. An assistant provides specific, planned assistance to a leader, and receives some form of compensation (free admission, lift ticket, or reduction in price). A volunteer provides unplanned assistance to a leader, on an as requested or volunteer basis, without compensation.
ANY trip or activity you led for the club, even if it was cancelled or you had to relinquish leadership (please describe the situation in an attachment), should be listed here. ANY planned and unplanned assistance you provided for a trip or activity should also be listed here. If you were a leader, please specify the program sponsoring the activity. If you assisted or volunteered, please specify the leader. In all cases, please specify your position on the trip or activity (Senior Leader, Junior Leader, Assistant Leader, Coordinator, Model, Instructor, Skipper, etc.)
