Dear Parents:
The New Jersey Department of Education, Office of Special Education is required to conduct an annual survey regarding the extent to which schools facilitated parent involvement in their child’s special education program. The enclosed survey has been sent to you as part of a statewide effort to collect information that will allow the New Jersey Department of Education to enhance special education programs and services (including speech-language services) and to improve educational results. Each state is collecting similar information as part of the State Performance Plan/Annual Performance Report that must be submitted to the United States Department of Education, Office of Special Education.
Your participation in this survey is completely voluntary and your individual survey responses will remain confidential. The survey will take about 5 to 10 minutes to complete. It is important that every parent take this opportunity to respond to the survey so that we will have enough information to determine areas of strength and areas that need improvement in order to increase and enhance parent involvement and outcomes for students. The survey was sent in prior years, but this year’s survey is being continued as an ongoing part of the state’s commitment to special education. I would greatly appreciate your assistance by completing the survey today and returning it in the enclosed postage paid envelope.
