Nursing Assistant /
RESPONSIBLE TO: Responsible to Home Manager
JOB PURPOSE:
The Nursing Assistant will assess in partnership with the person living in the homethe support needed and implement the planof support that has been agreed with support of the management team.
Report to theHome Manageron a continuing basis any changes noted in people’s condition/support by process of evaluation.
The post holder must participate as part of the care team and be an active member of the multi-disciplinary team within the Home and be willing to develop their support and management skills.
Main Responsibilities:
  • On a daily basis take direction from The Registered Nurse
  • As part of The Nursing team, together with the person living in the home and their families, ensure that their needs are supported. Develop a plan of support to ensure the delivery of the highest standard of support and service appropriate to those needs.
  • Implementthe planned support and ongoing reviews, reporting back to the Nurse in Charge any changes or concerns.
  • Ensure all people living in the home have up to date and agreed risk assessments andsupport plans which detail the interventions necessary; evaluating and reviewing the care plans in line with policy, to ensure people’s needs are being satisfied, in consultation with a registered nurse.
  • Keep accurate records in line with The Summerhill Group procedures e.g. drug administration, accidents and incidents, care planning, food & fluid charts, fundamentals of care daily records (note this is not an exhaustive list) in an accurate and timely fashion using manual or computer system as appropriate.
  • Participate fully in the overall care of the people who live in the home, ensuring quality of life through effective support. This includes encouraging the people living in the home if they chose to do so, to participate in recreational activities to promote a sense of fulfilment and belonging.
  • To undertake medication ordering and replenishment of supplies using the appropriate identified systems with the pharmacy and general practitioners.
  • To administermedication to people who have residential needs and Nursing Needs under the supervision of the Registered Nurse.Appropriatetraining must have been undertaken.
  • Give daily reports at handover with relevant members of the Nursingand staff team
  • Liaise with multi-disciplinary teams and arrange visits as necessary
  • Ensure people living in the home receive the highest standard of support by providing effective day to day supervision and communication with other members of the staff team
  • Ensure all of the staff team recognise the importance of making relatives, visitors and care professionals feel welcome by promoting the ethos of the home
  • Take the lead on all new admissions coming into the home. Ensure all people and their families receive and understand the welcome pack, and the person coming to live in the home is welcomed and settled within their new home.
  • To give new members of care staff an induction to the Home, as covered in the group’s induction programme (which encompasses the Care Council Code of Practice) and to work closely with new members of staff in order to make them feel comfortable and welcome as part of the team.
  • To undertake basic observations of people living in the home once competence has been checked by a trained nurse.
  • To undertake specialist clinical training as appropriate
  • To liaise with Nursing team regarding what diets and nutritional support people may need
  • Attend all statutorytraining sessions making certain that practices reflect the learning outcomes of the training. Keeping a reflective diary of how learning has influenced practice. Attend any other training sessions that The Summerhill Group deem applicable to the role.
  • Ensure that all training in conjunction with the group policies are understood and are adhered to.
  • To support and lead care staff in ensuring residents participate in recreational activities as they wish to facilitate the person centred approach.
  • To take responsibility for managing environmental factors within the nursing home in relation to health and safety, food hygiene, fire precautions and emergency procedures, infection control and emergency aid.
  • To act in accordance with the legal and statutory requirements relating to practice and the Homes’ policies and procedures.
  • Actively participate in staff meetings, communicating and helping to resolve any concerns that may have been raised whilst working with the people living in the home or other team members.
  • Maintain confidentiality of information regarding people living at the home, visitors and staff team
  • Undertake any other duties or training consistent with the role, including some or all of the Additional Tasks set out in the Summerhill Group Team Leader Nursing Policy (as amended from time to time), and to be assessed for competence in those tasks as required
  • Take responsibility for personal development by keeping abreast of development in the field of caring for older people.
CLINICAL SKILLS – Health and Wellbeing
  • Undertake, record and follow guidelines for the tasks for which you have received appropriate training e.g. catheter/ stoma care, diabetes awareness, basic wound dressings.
  • Urine/ stool analysis and preparation of specimens for investigation by the pathology
  • Measuring and recording following physiological measurements in routine presentations:
  • Blood pressure
  • Pulse rate
  • Temperature
  • Height and weight – body mass index
  • Venepuncture
  • Oxygen saturation
  • Prepare and maintain environment and equipment before, during and after care interventions
  • Assist in raising awareness of health and wellbeing and how it can be promoted
  • Communicate effectively and support effective communication
HEALTH AND SAFETY

Employees are required to co-operate with management to enable the Home to meet its’ own legal duties and to report any hazardous situations of defective equipment.
The above duties may vary from time to time without changing the general character of the post or the level of responsibility held. Such variations are a common occurrence and cannot of themselves justify a post re-grade.

Experience:

Qualified to NVQ Level 3 in Social Care/equivalent / √
