Joining the Swim Team Includes an Annual Fee and Session Fees

Dear Parents,

Welcome to the Germantown swim Team. In addition to the monthly swim meets, there are several activities we try to do together as a team. We have a kick off party at the beginning of the season in September, Halloween Activities, A Christmas Party, and our awards banquet in the Spring. All of these activities are announced through team emails.

Included in this packet you will find an explanation of fees, a fee chart, detailed instructions on how to register online, practice schedule, an explanation of USA swimming coverage, and other ways to connect online.

Explanation of Fees—

Joining the swim team includes an annual fee and session Fees :

·  The annual Registration fee of $225 covers your swimmer’s USA registration. Our swim club, coaches and swimmers are covered by USA swimming insurance. In order for the club and coaches to be covered, all swimmers practicing or competing in swim meets must be registered with USA swimming. USA swimming also provides additional coverage for its swimmers (see attached). Of the $225, $70 goes to USA swimming, and $45 goes to the City of Germantown for pool usage when Racquet Club pool is down. The remainder of the fee covers the team t-shirt, swim cap, and team parties and coach’s travel. (This fee is reduced for the period of March 1-August 31 to $115)

·  The second set of fees is based on the swim group your child is assigned to, and is also paid directly to the Germantown Swim Team Parent Association. It is requested that when you register that you assign a credit card to your account to charge these fees to that card on a monthly basis. (the fee schedule is attached). When registering, please keep in mind that you are committing for a season at a time. Seasons are as follows: Fall (Sept. – Nov.), Winter (Dec. – Feb.), Spring (March – May), Summer (June – August). Swimmers wishing to take a session off must notify GSTPA thirty (30) days prior to the end of that session. These fees cover coaches’ salary and pool usage.

·  Completed Online Registration (instructions provided)

·  $225 registration fee (non-refundable)**. Make check payable to GST

·  Copy of swimmer’s Birth Certificate

·  Signed Team Release and Waivers Form (included in this packet)

·  TN Concussion Form (found under documents tab on team website-signature and initial page must be printed)

·  Germantown Athletic Club session fee (2017-2018 fee sheet attached)—visit the GAC front desk to make this fee payment.

Registration Instructions

For New GST Families

1. Go to gstswimming.com, click on the START REGISTRATION tab, which can be found on the left side of the web page.

2. Review the Registration Requirements and print the necessary forms. Click Continue.

3. Click Create New Account, enter your email address and birthdate. Click Continue.

4. 2017-2018 GST registration page will open. Fill in all fields.

Please Note: Fill in the field marked “Alternate Email 1” with your primary email address , to receive team communications. Click Continue.

5. Click the Add Member tab on the top right.

Enter your swimmer’s information.

For Location: Select The Racquet Club of Memphis

For Register to This Group: Select the appropriate Group (Bronze, Silver I, Silver II, Gold, Senior). Click Add.

If you have more than one swimmer, repeat the process. Then proceed to the next step.

6. Read all electronic documents and, if you agree, check the boxes. (Digital photography and Email are optional). Click Continue.

7. Make sure all information is correct. Select method of payment as Credit Card or Check. (To change the method of payment click on drop down box). Click Proceed to Checkout.

8. The total charges will appear at the top of the page. If you agree click Submit Registration.

If paying by check make payable to Germantown Swim Team. Checks and other required registration forms can be placed in the blue box on the pool deck by the bulletin boards, or they can be mailed to:

9. Registration is now complete. You will receive a Registration Receipt by email. Once payment has been received you will receive an email to set up your personal account on our website, and an email to verify your email address. Go to your account online and check your email accounts and phone numbers to confirm they are verified.

10. Please remember, there are two payments for separate fees, $225.00 which is the team registration fee and paid directly to Germantown Swim team and swim session fees that are paid monthly.

USA Swimming Coverage

Membership Protection

USA Swimming’s policy for membership protection is:

• To provide safety education for its membership;

• To provide excess accident medical protection for USA Swimming members who may suffer injuries while participating in insured activities;

• To provide evidence of financial responsibility so that USA Swimming clubs can conduct insured activities.

To implement this policy, USA Swimming has adopted two major programs:

• Excess Accident Medical Protection

• General Liability & Excess Insurance

These programs are intended to provide reasonable protection for USA Swimming athletes, non-athlete members and clubs.

EXCESS ACCIDENT MEDICAL INSURANCE DESCRIPTION OF COVERAGE

Policy Term of Coverage: January 1, 2017 to January 1, 2018

Insureds: Members of USA Swimming

When Coverage Starts: Coverage is a benefit of membership in USA Swimming and begins upon receipt of the completed membership application form with appropriate fee and acceptance by the LSC

Registration/Membership Chair or its designee.

When Coverage Is In Effect:USA Swimming members are covered while participating in a USA Swimming supervised, sponsored, sanctioned or approved event including:

· Competitions and meets;

· Organized practice sessions;

· Approved social and fund raising activities;

· Travel to and from competitions, meets, events, organized practice sessions, approved social and fundraising activities at the direction of a coach or club board of directors;

· Observed Swim Meet events for USA Swimming Officials only

Coverage Outline:

When covered Injuries result in treatment by a Legally Qualified Physician beginning within 90 days of the accident, the Medical Expense incurred in excess of the Medical Deductible, if any, will be paid. Benefits will not exceed a maximum of $25,000.00. Benefits must be Medically Necessary and shall not exceed the Usual and Customary charges in the geographic area where treatment is performed. Only covered Medical Expenses incurred by the Insured within 52 weeks from the date of the accident are covered.

Benefits:

· $25,000.00 maximum per occurrence for Accident Medical Expenses. Eligible Medical Expenses are: (a) Treatment by a Legally Qualified Physician; (b) Care or services from a Hospital or Ambulatory Surgical Center; (c) Services from a registered graduate nurse (RN or LPN) not related to the Insured by blood or marriage; (d) Professional ambulance service; (e) Orthopedic appliances; (f) Injuries to sound and natural teeth.

· $1,000.00 maximum per occurrence for Chiropractic or Physical Therapy treatment/expenses

In accordance with the GST Mission Statement and Vision Statement, and in association and compliance with USA Swimming, and State of Tennessee Law, GST has adopted several policies and guidelines aimed at the protection of our children, coaches and parents. The intent of these policies and guidelines is to communicate and provide parameters of what is expected of all involved with GST. Where possible, we have provided age appropriate versions of certain policies. It is a requirement of participation with GST that all registered GST Swimmers, their parents and coaches acknowledge all of these policies and guidelines.

The $225 early registration fee must be paid when the swimmer joins the team

**Seasonal fees are paid to GSTPA through the team website. It is requested that each family attach a credit card to their account in order to make the monthly drafts for swim team fees. Each swimmer is committed financially from the time they start training with the team through the end of that session. Swimmers wishing to take a break must notify GSTPA no later than thirty (30) days prior to the end of the session in order for their account to be suspended without having to pay for the next session.
