Job Title: / Urgent Care Senior Physiotherapist– Band 6
Salary: / £26,302to £35,225 per annum (pro rata)
Reports to: / Team Manager
Job Purpose: / The post holder will work as a key member of Rapid Response, andbe part of a rotation into REACTIn-Reach teams to provide specialist assessment and intervention to adults 16 years and over in theacute and in the community setting. The teams work flexibly toprevent unnecessary hospital admissions by channelling patients to appropriate services.

The promotion of shared decision making as well as personalised care planning throughout the acute and community aspects of our service to support and enable patients to self-manage in their own home environment. To work alongside the team managers to ensure the service has processes in place to ensure that clinical governance of the service is being met and we continue to be innovative by engaging with Patient and Public Involvement to make changes and improvements on the way we work.
Key Responsibilities: /
  • To have a dual role as part of the REACT In-Reach rotation into the BRI and as a senior Physiotherapist within the Rapid Response team treating people in their own homes
  • The post holder will undertake a holistic assessment of a variety of disorders, diagnose and plan treatment as part of a multi-disciplinary service approach in order to provide a co-ordinated package of care and safe discharge from the service.
  • To collaborate and negotiate with patients, carers, family and when In-reaching, the ED staff, in identifying appropriate discharge options and formulating a safe management/discharge plan.
  • To work flexibly within the REACT In-Reach service to facilitate discharge for appropriate patients with a hospital stay of 5 days of less and to work within Rapid Response in the community to prevent admission and facilitate patient flow and a robust service across teams.
  • To use specialist skills and techniques in assessment of cognitive and physical function, drawing from individual and interdisciplinary knowledge; assess, prescribe, provide and order appropriate equipment within a set budget or to refer on as necessary (e.g. to Care and Repair or Social Services for ramps, rails, adaptations etc).
  • To plan, co-ordinate and integrate working with other agencies (e.g. Intermediate Care Teams, Primary Care Agencies, Social Services and Voluntary Agencies) to facilitate safe discharge and provide rehabilitation.
  • To work towards achieving BCH/ Emergency department targets for REACT In-Reach, providing smooth and efficient facilitation of discharge when working in this setting.
  • Ensure high quality patient care, implement best practice
  • Work alongside the operational manager to ensure the delivery of performance indicators and targets.
  • Actively and positively contribute to the values of Bristol Community Health
  • To provide clinical supervision to nursing / therapy students in partnership with the University; supervise junior staff members, and rehabilitation assistants carrying out delegated physiotherapy exercise programmes with service users.
  • Actively promote inter-disciplinary collaboration by working closely with other team members and by having a thorough understanding of their roles and the role of the Urgent Care
  • Contribute to the in house training programme by undertaking relevant teaching sessions as well as support of junior staff with their competency programmes.
  • Adapt treatment programmes to suit the patient’s home environment.
  • Understand the constraints and flow of patients through the maximum 7 day remit of the Rapid Response Team.
  • Work as an autonomous practitioner
  • To provide support for the Lead Physiotherapist and ANP to ensure the smooth and efficient running of Early Discharge Liaison service
  • Maintain and complete appropriate and timely patient clinical records in line with professional and NHS Trust guidelines

Experience: /
  • Post registration experience in community and elderly and/or disability settings, orthopaedics, respiratory & neurology
  • Experience of community care and the interface with secondary care.
  • Experience of having undertaken clinical supervision, audits and patient involvement exercises.
  • Experience of multi-disciplinary team working.

Knowledge and qualifications: /
  • Registered Allied Healthcare professional
  • Degree level qualification or equivalent
  • Relevant technical knowledge to diploma level or equivalent
  • Knowledge of community services and Intermediate Care
  • Knowledge of team work/dynamics in relation to multidisciplinary working
