Note: Cost per allocation $815.00 inc GST. One allocation is equal to 25m2. Maximum one allocation per team, plus additional one allocation per association (for physio) permitted. Each allocation includes flooring and ONE TABLE only in each team tent.

5. Association Secretary Sign Off
By signing and submitting this form you acknowledge on behalf of your Association that your Association will be immediately liable for the total payable amount, in full, for all tent allocation fees and associated equipment fees regardless of whether:
(i) the 2017 Samsung State Age Championships proceed as scheduled or at all; or
(ii) inclement weather or other conditions means that your Association do not have any access to the tents during the 2017 Samsung State Age Championships due to safety concerns arising from inclement weather or other adverse conditions.
You understand your Association will be invoiced for the total payable amount as shown over page and acknowledge this amount must be paid in full 7 days prior to the commencement of the Championships.
You acknowledge that your Association is not permitted to erect your own tent or structure, nor can the Association sub contract the erection of team tents and structures on your behalf. You also acknowledge that parents may bring their own tents to erect, but the erection of these tents is subject to weather and availability of space in the designated parent tent area on the day. No parent tents are permitted to be erected prior to 6.00am on the first morning of the Championships.
