Instructions:
1. This form is also applicable to Degree program transfer.
2. Please refer to the CHU Regulations Governing Department Transfer and qualification requested by the intended departments prior to applying for a transfer.
3. Students applying for a transfer shall fill this application form and attach his/her
transcripts for semesters to this form, and shall submit the documents to the Division
of Register of the Office of Academic Affairs for process. (The transcripts can be
obtainedfrom the coin-operated slot machine.)
4.Prior to submitting the documents to the Division of Register of the Office of Academic
Affairs for process, this form shall be singed by the mentor, chairman, and dean of the
applicant’s original department (degree program) or institute. The application documents
shall be submitted before the deadline stipulated by the Office of Academic Affairs.
Otherwise, the applicants shall be disqualified for a transfer.
5. After archiving all the application documents, the Office of Academic Affairs shall
send the application documents to related admitting departments for the preliminary reviews.
6. The administrative staff in each department shall conduct a preliminary review of the applicants’ qualification according to department transfer standards stipulated by the department. After the preliminary review, the application documents shall be forwarded to the department’s review panel for further evaluation.
7. The results of the transfer review by the department shall be sent to the Division of Register of the Officefor archive, which will hold a meeting regarding department transfer for a secondary review.
8. The results of the transfer review shall be announced by the Office of Academic Affairs.
Those who are admitted by the admitting departments (first priority, first) shall not request
to go back to their original departments, and are not allowed to apply for a dropout or
temporary suspension.
