· Please include an Entry Report indicating proof of time by checking the “Include Proof of Time” box
· Please double check all entry times, genders, and ages before sending
Event Order:
· First session: swimmers aged 11 and older will compete in the morning session
· Second Session: swimmers aged 10 and under will compete in the second session
· Session start times will be announced once all entries have been received
Entry Eligibility:
1. Each swimmer may swim a maximum of 3 individual events and 2 relay events
2. Age as of October 1, 2016 determines age group of swimmer
3. Birth dates must be included with entries. No swimmer will be allowed to swim without a birth date
4. Swimmers must have competed in an event during the current season within a Regular Season League meet in order to swim the event during the Championship Meet. NT and time trial times will not be accepted.
5. Swimmers must be entered with a best, official time (proof of time) obtained in a League Meet, USA Swimming meet or YMCA meet within the current season. NT’s will not be accepted. If a swimmer has not achieved a legal time in an individual event, a made-up time will not be accepted. All entry times must have been achieved between September 1, 2016 and the deadline date, this includes relay entry times.
6. No late entries will be accepted, no exceptions. NO DECK ENTRIES

Entry Fees:
· $5.00 per individual event
· $8.00 per relay event
· Entry fee report and check due by March 13, 2017

Relays:
· Each team may enter 3 relay teams per relay event (three can place, but only two can score)
· A swimmer may age up 1 age group to participate in a relay, but CANNOT age down
· Relays must be of mixed genders (minimum of 1 female must be on each relay team)
Scoring:
· Individual events: 16, 13, 12, 11, 10, 9, 7, 5, 4, 3, 2, 1
· Relay events: 32, 26, 24, 22, 20, 18, 14, 10, 8, 6, 4, 2
Awards:
· Individual: 1-6 places will be awarded medals; 7-12 places will be awarded ribbons
· Relay: 1-6 places will be awarded medals; 7-12 places will be awarded ribbons
· High Point Awards will be awarded to each age group gender
· Runner-Up Awards will be awarded to each age group gender
· Girls and Boys Team High Point Awards will be awarded to each age group
· Team Trophies will be awarded to the top three teams in each division
· Participation medals will be provided to all swimmers
