Post: consultant in palliative medicine

Base: ACCORD HOSPICE

Applications are invited for this newly created 0.6 WTE substantive consultant post in Palliative Medicine. This post has been supported by the Board of Directors to support the evolution of ACCORD’s care and service provision and is in line with the hospice’s strategic direction of supporting patients regardless of their setting. The successful candidate will integrate with existing consultant colleagues and provide clinical excellence in Palliative and End of Life Care to the communities of Renfrewshire & East Renfrewshire (Barrhead, Neilston & Uplamoor).

The post will offer a real opportunity for the successful candidate to be involved in the provision of a successful modern consultant led service.

Applicants must have full registration with the General Medical Council, a licence to practise and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT in Palliative Medicine or specialist registration (CESR) or be within six months of confirmed entry at the date of interview. Non UK applicants must demonstrate equivalent training.

Introduction

ACCORD Hospice, an independent registered charity, has since 1992 provided a comprehensive range of specialist palliative care services to the people of Renfrewshire and parts of East Renfrewshire, a population of approximately 200,000. ACCORD Hospice has an 8 bedded in-patient unit, 15 place day therapy unit which also provides a variety of complimentary therapies. Through our multidisciplinary team we offer physiotherapy, occupational therapy, lymphoedema therapy, social work and patient and family and bereavement support. Our community services are led by a team of 6 Community Palliative Care Clinical Nurse Specialists who work closely with the local community teams. Education is coordinated by our lecturer in palliative care and ACCORD is actively involved in the teaching of many University courses. Links exist with the Medical and Dental Schools at the University of Glasgow with regard to under and post graduate teaching, Nursing and Paramedical courses at the University of West of Scotland and Glasgow Caledonia University, all with whom we have excellent working relationship.

ACCORD hospice maintains good working relationships with NHS Greater Glasgow and Clyde through regular meeting, its participation in the local Palliative Care Managed Clinical Network and one of our Consultants in Palliative Medicine holds a joint NHS appointment between NHS Greater Glasgow and Clyde and ACCORD. Opportunities exist for the successful candidate to participate in the Hospital Palliative Care support team sessions.

Position: Consultant in Palliative Medicine

Responsible to: Professional accountability to the Medical Director or Chief of Medicine RAH NHSGGC Health Board.

Managerially accountable via the hospice Chief Executive to the Board of Directors of ACCORD.

Hours per week:0.6WTE based on 6 programmed activities

Basis of Contract: The employment contract will full Terms & Conditions as supported by NHS Greater Glasgow Clyde Health Board (honorary), although fully funded by ACCORD hospice.

Annual Leave:6 weeks + 10 public holidays (pro rata)

Duties of the Post

Clinical

Providing and being responsible for the clinical care and ongoing management of patients requiring specialist and general palliative care within the In-patient unit of the hospice, including advice and support of family and carers.

Provide clinical care, advice and support to day hospice and out patient services.

Provide specialist medical advice and support to the team of Community Palliative Care Clinical Nurse Specialists, local General Practitioners, District Nurses and, where requested, visit patients at home.

Work closely with other specialist colleagues to maintain and develop specialist palliative services in the hospital and community,

This appointment is a part-time post comprising of 6 Programmed Activities, 5 Direct Clinical Care (DCC) sessions and 1 Supporting Professional Activity (SPA) session per week.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

On call

1st on call is provided by GP sessional staff to achieve a 1:7/8 rota.

2nd on call Consultant rota is 1:5 shared with consultant colleagues of Ardgowan and St Vincent’s hospices providing out of hours advice and support to all three hospices as well as telephone advice and support to the hospitals of Clyde.

Continuing Medical Education

The post holder will be expected to participate in continuing medical education as advised by the medical Royal Colleges.

Research

The person appointed may participate in research projects within ACCORD Hospice or undertake research on his/her own account.

Multidisciplinary Liaison

Liaison will be expected with medical, allied health professionals, social work and nursing colleagues on the management of patients within the service, ensuring adequate communication between all members of the multidisciplinary team including the patient, their carers and the community health and social care teams.

Participates actively in the local palliative care network with colleagues from other NHS and Independent specialist palliative care providers.

Quality Issues

The post holder will be expected to contribute to the setting and monitoring of quality standards within the various services of the hospice.

Active participation in the Hospice Clinical Governance agenda is expected.

Audit and Clinical Effectiveness

The post holder will be expected to contribute to the audit and evaluation programme of the hospice.

Staff Training and Teaching

The post holder will be expected to support junior doctors in training who may be attached to the hospice and contribute to the teaching of undergraduates and postgraduates as requested.

Participate in the multi professional education programme of education and training for hospice staff and where requested for staff in the local hospitals and community setting.

Management

The post holder will be expected to work with the Chief Executive and Senior Management Team to support in the efficient running of the Hospice services.

To report on medical matters as and when required to the Board of Directors.

To collaborate with the appraisal and revalidation of Hospice medical staff and providing mentorship as required.

Personal Development

There is an annual appraisal of medical staff which contributes to revalidation. Consultants are required to participate fully in the appraisal processes and bring to the attention of appraisers any issues which may adversely affect performance.

The post holder will have a requirement to keep his / her knowledge and skills in palliative medicine up to date; ensuring that agreed study leave complies with the requirements for continuing professional development and revalidation to maintain their Licence to Practice with the GMC.

Cover for Holidays

So far as is consistent with the proper discharge of the above duties the post holder undertakes to deputise from time to time for absent colleagues. The post holder undertakes, exceptionally, to perform additional duties in occasional circumstances. The post holder undertakes, exceptionally, to be available for such irregular commitments outside normally rostered duties as are essential for continuity of patient care.

Support for the Post Holder

Support includes office accommodation, including IT facilities and medical secretarial support.

Access to clinical supervision will be provided to support the post holder if required.

TERMS AND CONDITIONS OF SERVICE

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.

£76,761 to £ 103, 490 per annum (pro rata)
New Entrants to the NHS will normally commence on the minimum point of the salary scale, (dependent on qualifications and experience). Salary is paid monthly by Bank Credit Transfer.
HOURS OF DUTY / 24 hours (6PA's)
SUPERANNUATION / New entrants to NHS Greater Glasgow and Clyde who are aged sixteen but under seventy five will be enrolled automatically into membership of the NHS Pension Scheme. REMOVAL EXPENSES / Assistance with removal and associated expenses may be given and would be discussed and agreed prior to appointment.
