We are looking for a Care Coordinator to join our Care OperationsTeam at our Leatherhead office.

Crossroads Care Surrey is a registered charity that provides much needed respite breaks to support Carers of adults, children and young people with care needs. We provide highly trained care staff to take over the caring responsibility for a few hours a week, enabling the Carer to have some precious time to themselves.

We improve the quality of life for Carers and the people for whom they care by enabling them to take a break from their caring responsibilities.

Our services operate throughout Surrey and include; our Core Service, which is primarily funded by Surrey County Council and the NHS, provides Carers witha 3 hour weekly respite break. This usually takes place on the same day and at the same time each week. Part of the Care Coordinator’s roleis to ensure that the same Carer Support Worker (CSW) attends each visit so that the family gets to know them and in turn the CSW has a clear understanding of the family’s needs, Social Clubs that give Carers an extended break and a variety of self-funded Care Packages that offer Carers overnight and weekend respite breaks at a low cost. In partnership with the NHS we also provide an unlimited end of life service for those caring for someone in the last twelve months of their life.

Crossroads employ professional staff that are trained to the highest standards and as an organisation we are compliant with the Care Quality Commission, and other required regulatory bodies.

Having fully trained and competent staff is important to us and you will be supported by a programme of continuous training and development.Our Care Coordinators are encouraged and supported to see their work in terms of helping people to achieve their personal goals and not just to perform set tasks. In addition to training and development we offer:

Criminal convictions

This appointment is subject to a satisfactory enhanced disclosure from the Disclosure and Barring Service (DBS). Where appropriate this check will also show whether a person is barred from working with vulnerable adults and / or children. Because of the nature of this work you need to provide information about any criminal record you have, spent or unspent. This includes cautions, reprimands and warnings.

Amendments to the Exceptions Order 1975 (2013) provide that certain spent convictions and cautions are ‘protected’ and are not subject to disclosure to employers, and cannot be taken into account. Guidance on criteria on the filtering of these cautions and convictions can be found on the Disclosure and Barring Service website.

If you do have any previous or outstanding convictions, cautions, reprimands or warnings that are not “protected”, you will only be asked to supply details if you are invited for interview. JOB DESCRIPTION

jOB tITLE:Care Coordinator

Reporting to:Deputy care operations manager

Salary:circa £23,500 per annum

LOCATION:Leatherhead Office, 121 Kingston Road.

Purpose

  • To assist in the management of Crossroads Care Surrey to provide a respite breaks service for carers in line with the aims, objectives, policies and procedures of the organisation.
  • To contribute to the promotion and the business growth of the organisation.
  • To maintain positive work ethics throughout the branch, supporting Management & Crossroads Care Surrey Board decisions
KEY Responsibilities
  • To manage the service delivery of Carer Support Workers (CSW’s) to ensure it is in line with agreed standards and rota within a designated area.
  • Carefully matching CSW with client
  • Ensure care plans are implemented and monitored according to the changing needs of the carers and the cared for person.
  • Identify and report incidents, accidents and near misseswithin the timeframe and guidelines of Crossroads Care Surrey Safeguarding procedure.
  • Maintain recording systems in accordance with the care operations team process and procedure.
  • Lead and manage 6 weekly CSW meetings for area
  • To ensure care hours are delivered to high standard as per contract specification.
  • To signpost to other agencies as appropriate.
  • To be part of the‘out of hours’ on call team to ensure cover
  • To assist in recruitment of CSWs for this role’s designated area.
  • To carry out the induction and training (shadowing) of new staff.
  • To ensure all training meets legal standards and the Carers Trust requirements
  • To carry outCSWs supervisions and appraisals.
  • To facilitate and ensure the smooth day to day running of the office.
  • To work with the Care Operations Manager to ensure compliance with Care Quality Commission and other relevant regulatory requirements.
  • To consult with carers to identify the levels of satisfaction and future needs.

General Duties:

  • To support business relationships with all stakeholders and exploring business development opportunities which benefit carers and the organisation as required.
  • To attend meetings as directed by the Care Operations Manager
  • To be a positive Ambassador for Crossroads Care Surrey and the branch
  • To follow Carers Trust Code of Conduct.
  • To assist the Care Operations Manager to ensure that the organisation is compliant with health and safety legislation.
  • To work within the philosophy and policies of the organisation.

Person Specification

Essential / Desirable
Education
  • QCF level 2 in health & social care or equivalent (prepared to work towards level 3)
  • Competence in numeracy and literacy
