A. Bouwhuis Fitness For Life Disclosure Bingham High
The mission of Life Fitness is to provide each student with an understanding of the following fitness components: cardio-respiratory endurance, muscular strength, muscular endurance, flexibility, and body composition. This will be accomplished through circuit training, tri-fit testing, the use of Polar heart rate monitors, and various sport activities.

I. Attendance & Make-up
a. Regular Attendance: 10 points will be awarded each day the student is in class, on time, and dressed appropriately for activity. Scores will be reduced to 5 points if tardy or proper clothes or shoes are not worn.
b. Class Credit: The award of credit in this class is subject to the conditions outlined in the Bingham High School Attendance Policy. Absences in excess of 4 per class per quarter that are not made up or waived through the appeal process will result in loss of course credit regardless of the course grade.
c. Makeup: make-up work will be offered for excused (E) absences only and you are allowed only 2 make-ups. A sluff (Z), guardian (G) excused absence or unexcused (A) absence cannot be made up. Students are allowed to make up missing points from an absence by doing a heart rate monitor workout. Since this is a participation class you will not receive a passing grade if you miss more than 60% of the time in a quarter.
II. Grading Procedures
a. Your grade for this class will be weighted as follows
50% Heart Rate Monitor Workouts
20% Assessments (Tri-Fit, 1 mile fitness run, 1 rep max’s, total weight improvement)
15% Attendance
10% Tests
5% Assignments (disclosure, bowling receipt, combination lock, & others)
100%
b. Grading Scale
94-100%= A 80-82%= B- 67-69%= D+
90-93%= A- 77-79%= C+ 63-66%= D
87-89%= B+ 73-76%= C 60-62%= D-
83-86%= B 70-72%= C- 59% & below= F
III. Class Fees:
i. $7.00 fee will be assessed for a heart rate monitor strap rental.
ii. $24.00 fee will be assessed for bowling three times during the semester.
IV. Class Expectation: Rules & Consequences
a. Every student is expected to:
i. stay on task the entire class period
ii. demonstrate their best effort
iii. show respect to classmates (i.e. cooperation, appropriate language,)
iv. remain with the class at all times (do not leave gym, fitness room, or wander the gym halls, or go inside when participating in outdoor sports without permission)
v. use the restroom before or after class or during dress times (use of the bathroom after roll call and prior to dress bell will result in a loss of participation points)
vi. dress appropriately for class each day (i.e. Gym shorts or sweatpants, shirts with sleeves and which cover the midriff and chest, sweatshirts if necessary for outdoors and sturdy tennis shoes)
vii. bring a master combination lock and keep their personal items locked up at all times (combinations should be reports to your instructor)

VI. Special Circumstances
Students with special learning needs or extenuating circumstances for absences should initiate a conversation with the instructor to discuss and resolve those issues.
