  1. PPG Sign Off

Report signed off by PPG: YES/NO
Date of sign off: 25/03/15
How has the practice engaged with the PPG:
How has the practice made efforts to engage with seldom heard groups in the practice population? We do publicise the PPG through the website and through a newsletter.
Has the practice received patient and carer feedback from a variety of sources? From ‘Friends & Family’ and also ‘I Want Great Care’. Also the PPG members feedback as patients and carers.
Was the PPG involved in the agreement of priority areas and the resulting action plan? Yes
How has the service offered to patients and carers improved as a result of the implementation of the action plan? The action plan itself is still in its infancy. The practice has however looked at procedures in place with regard to any areas of concern raised by the PPG.
Do you have any other comments about the PPG or practice in relation to this area of work? It is a very supportive group and we look forward to working together to develop the practice and eventually get a new building.
