ATHLETIC EVENT/SOCIAL PERMISSION SLIP
Dear Parents:
The SEMS softball team will host a home game on Thursday (September 21st). This event will begin immediately after school and will conclude at 6 pm. Tickets are $2 and will be sold at the gate. A signed permission slip is a necessity for students to attend.
- Students that have been suspended or are in ISS will not be able to purchase tickets or attend this event. Also, students who are absent on the day of the event will not be able to attend. Students will follow school and district policies when on school campus for events.
- The students will be disciplined according to the code of conduct for inappropriate actions at any school event, and the principal reserves the right to refuse the student from future after school events.
- Students must remain in the designated area for the activities. Students attending sporting events must remain in the bleachers and not play behind the bleachers, in the field, behind the dugouts, etc.
- Please be sure you pick up your child at the designated time. If a child is picked up late from activities multiple times, the principal reserves the right to refuse the child from future after school events.
- After school athletic events and socials are meant to be enjoyable and memorable events for students; therefore, their safety and the safety of others will remain our focus and will be our effort to maintain.

I, ______, Parent/Guardian of ______, agree to allow my child to attend the SEMS softball game and I realize that my child will be subject to all school rules while attending this activity. I understand that the games will end at 6 pm and I will pick my child up on time.
