Materials Ordering Update • Texas Assessment Programs • STAAR EOC 2012

Directions:

1.  All schools must complete this form. This is a snapshot of your total number of students to be tested as of the date this form is completed. Save a copy of the electronic form to your computer; open it in Microsoft Word and complete. Use one participation form per campus.

2.  Type the number of test booklets needed (number of students to be tested) by grade level. Fill in the exact numbers needed for the assessment programs.
DO NOT round up your counts.

3.  Email the completed form to the Testing Materials Center no later than 5 P.M., January 13, 2012 by doing the following:

a.  Click on which will open a blank email; if not, open an email and type the address.

b.  Use the “paper clip” icon in the menu to find and attach your completed order form to the email.

Make sure that “TAKS ORDER for YOUR CAMPUS NAME” is in the email SUBJECT line.
