Liability Waiver
Having been informed of the Organization of the Hi-Point Leagues, Bellefontaine, Ohio, to provide supervised baseball games for boys/girls, I/we, the parents of the above named candidate(s), do hereby give my/our approval to participate in any and all of the activities during the current season. I/we do assume all the risks and hazards incidental to the conduct of the activities, transportation to and from the activities; and I/we do further hereby release, absolve, indemnify, and hold harmless the Hi-Point Baseball Association, City of Bellefontaine Joint Recreation District Park Board of Trustees, their employees and volunteers, the organizers, sponsors, and the supervisors, any and all of them. In case of injury to the above named candidate, I/we hereby waive all claims against release from responsibility, any person transporting the above candidate to or from the activities.
We, the parents of the above named candidate, our child, do understand all of the above; and will furnish a copy of the above named candidate’s report card, along with evidence of our child’s school district, to officials of the Hi-Point Baseball Association, if necessary.
