Please address the following questions in your institutional grant request. The format outlined should be followed in the preparation of your proposal. The number of pages is not to exceed those indicated in parenthesis. If you need assistance, please call Page Wolf in the Professional Development Center (x2446).
Proposals must be double-spaced typewritten.

Departmental Support
Please have your direct supervisor (Director, Dean, etc.) email a letter of support of your project. This letter should indicate that the project has support and that all other internal funding opportunities have been pursued. Please list the source and amount of funding (see p. 7). If you are requesting release time, overload pay, or a stipend, a letter from an appropriate administrator needs to be submitted as well.
Support letters should be emailed care of the Professional Growth Commission to and addressed to the CLC Foundation Grants & Scholarship Committee.

Describe the specific sequence of activities that will be carried out during the grant period and the implementation timetable for the project activities.
Project Personnel
List all persons who will work on this project. For each person, indicate what role he/she will fill on the project.
Impact:
a. Short-term Impact - During the period of funding, how many people will be affected by this project and what benefits will they derive from it?
b. Long-term Impact - If it is successful, how might this project affect the college, its operations, its students, and/or its personnel over the next 2-3 years? How might what is learned from this project be more broadly applied? Do you foresee any future "spinoffs" of this project?
c. Personal Impact - What do you personally expect to learn from your involvement in this project, and how will that knowledge impact your professional competency, job satisfaction, etc.?
Evaluation Plan (one page)
How will you be able to determine whether or not this project is successful? Describe the information you plan to collect which will enable you to verify the degree to which the project has met its goals and objectives.
Budget
For each type of expense, describe in detail what you need, and how much it will cost. Provide the calculations you used to arrive at the cost. Also, please describe how funds will be used for the complete project.
Personnel
Examples: release time for project director to oversee project, @ $$ per hour x N hours (mandatory for release time requests)
N student workers needed for X work on project, each @ $$ per hour x N hours per week x N weeks
Travel (should be used primarily for local travel by staff on project business, or student travel necessary for project, say a field trip)
Example: local travel via auto for project director to schools, @ .55 cents per mile x N miles per week x N weeks
Equipment (any durable item needed for project, unit cost $5,000 and up)
Example: 2 automatic book check-out machines for the Library to increase efficiency and reduce lines, @ $5,000 each = $10,000
Supplies (any expendable item needed for project, unit cost less than $5,000. This may include low-cost laptops, printers, tablets, etc. – although their use should be justified for the project and some means of inventorying and tracking them should be given.)
Examples: Printing/Reproduction, @ .10 per page X N pages
Teaching materials needed for class involved in project (books, software, samples, artwork, what have you), @ $$ per material X N materials of the type
15 Google Nexus 7 tablets for innovative pedagogical approach in Class Z @ $249 each = $3,735 (tablets to be inventoried on arrival and kept locked in departmental cabinet outside of class time)
Contractual Services (any payment to be made to any individual or firm external to the college, but whose expertise or resources are needed to complete the project)
Example: Expert Educational Evaluator to collect and analyze data on project’s progress and success as defined in proposal, @ $200 per hour x 20 hours = $4,000

CLC Budget Impact
Provide a brief summary of the impact on existing college resources: what will need to be used in order to complete the project, in addition to the funds being provided by the grant (additional personnel, space, equipment, etc.). Indicate the sources of this support.
Example: 5 more Google Nexus 7 tablets are needed for project staff; the department will purchase them out of its own funds
