I agree to have the above described animal altered no later than by a licensed veterinarian of my choosing. I acknowledge that, depending upon my selected option for spay/neuter, SOS may bear all, some or none of the responsibility for all costs associated with the spay/neuter of my animal.

Furthermore,

1.  I understand that this spay/neuter addendum is an agreement that this dog will not produce a litter of puppies either as purposely bred or by accidental breeding.

2.  SOS has provided me with a list my options for spay/neuter and I agree to follow one of the noted options.

3.  I understand that a portion of the adoption fee, no less than $40, serves as a deposit to have this surgery performed. Depending upon the spay/neuter option I select, this deposit may or may not be refunded to me.

4.  I agree to forward proof of spay/neuter to SOS within seven (7) days of the completion of the surgery either via mail, email or fax.

5.  I agree that should the animal not be altered as agreed upon, I will return said animal back to SOS with no fee refund.

6.  I agree that if am I unable to comply with any of the medical requirements for my adopted animal, I will contact SOS immediately so that SOS can insure that such are completed in a timely manner.

7.  I certify that I have read and understand the terms of this Spay / Neuter Agreement. I acknowledge that failure to comply with these terms shall be considered a breach of contract and shall result in the immediate return of the adopted animal to SOS and payment of a minimum of $500.00 in damages. Additionally, I may be subject civil penalties.

8.  I agree to pay for ALL legal and/or court expenses should this contract be violated and it becomes necessary for SOS to pursue legal avenues to repossess adopted dog or to take any legal action for violation of this contract. For legal purposes the value on this adopted dog is no less than $1,500.

I have read and understand the above Addendum, and agree that it is part of the Adoption Agreement. I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with SOS. By affixing my signature to this addendum, I agree to ALL of the above conditions. The dog is not legally adopted until I have paid the adoption fee in full and submitted a properly executed Adoption Contract and this Addendum.

WHY DO WE SPAY AND NEUTER?

·  Neutering reduces the possibility of testicular tumors, prostate problems, perianal tumors and hernias.

·  Neutered males are less likely to escape a yard or "roam" to find a female in heat. This reduces the chance of them being hit by cars, getting into fights or lost.

·  Spaying females before their first estrous cycle reduces their chances of mammary tumors, ovarian cancer and uterine infection. The scent from unfixed females in heat can be detected by males more than 2 miles away, causing anxiety for the entire neighborhood.

·  Giving birth to a litter can be dangerous for your pet.

·  Spaying and neutering before sexual maturity often results in a temperament benefit. Males neutered early in life tend to be less aggressive and less likely to mark in the house. A spayed female does not have hormonal mood swings and can make for a superior companion.

·  There can be many serious risks involved with breeding and giving birth. The mother can develop complications and require expensive medical treatment, even resulting in death, leaving you heartbroken and with a newborn litter that needs round-the-clock care.

·  There is no need to breed - there are many lovely animals in shelters and rescues, over 25% of whom are pure breds, just waiting to find their furever homes.

SPAY/NEUTER OPTIONS

You have adopted an unaltered dog and have agreed to have the dog spayed/neutered by a certain date. The following spay/neuter options are available to SOS adopters. Please review these options and determine which will be the most convenient for you and make an appointment as soon as possible - some clinics book 3-4 weeks out. Please remember to forward proof of spay/neuter to SOS within seven (7) days of the completion of the surgery either via mail, email or fax.

Rhode Island

·  RI Community Spay/Neuter Clinic, 235 Elm Street, Warwick, RI 02888, 401.369.7297, www.ricsnc.org. If the surgery is done at this clinic, the appointment will be made under "Save One Soul" and the dog's SOS name, and scheduled as it works out with my schedule. For no extra charge, the clinic will provide pain medication administered in clinic, distemper/parvo and rabies vaccinations (as needed), and two days post-op pain medication. If the S/N clinic works for you, this is probably the most economical option. This clinic will insert a microchip at the time of S/N, at no additional charge, but I will need to provide the microchip elsewhere. If this option is selected, SOS will cover all expenses for altering.

·  Companion Animal Welfare Society, 88 West Warwick Avenue, West Warwick, RI 02893, www.cawsri.com, 401-615-2200. If the surgery is done at this clinic, the appointment will be made under "Save One Soul" and the dog's SOS name, and scheduled as it works out with my schedule. If the animal is due for vaccination boosters, this will also be done at this time at no additional charge. If this option is selected, SOS will cover all expenses for altering.

·  Titan Veterinary Services, 90 Airport Rd, Warwick, RI 02889, (401) 921-4546. If the surgery is done at this clinic, the appointment will be made under "Save One Soul" and the dog's SOS name, and scheduled as it works out with my schedule. If the animal is due for vaccination boosters, this will also be done at this time at no additional charge. You understand that microchipping is available at an additional charge. If this option is selected, SOS will cover all expenses for altering.

Connecticut

·  New Hope Clinic, 214 Canal Street Plantsville CT, 06479 Phone: 860-620-0325 Fax: 860-620-0359. If the surgery is done at this clinic, the appointment needs to be made under "Save One Soul" and the dog's SOS name, and can be scheduled as it works with your schedule. The clinic will administer pain medication at the time of surgery, and vaccinations (as needed) can be administered for an additional fee. This clinic will insert a microchip at the time of S/N for an additional charge, but you will need to provide the microchip elsewhere. If this option is selected, SOS will cover all expenses for altering; you will be responsible for other expenses. ACO’s, shelters and Non-Profit/501c3 organizations please call 860-620-0325 and press #2 - Rescue Organization Program – We provide discounted rates to rescue organizations to ensure that the animals they have in their care are sterilized prior to adoption.

Other

·  FOA Spay/Neuter Certificate. You may purchase a discounted spay/neuter certificate from Friends of Animals (FOA) at http://friendsofanimals.org/programs/spaying-neutering/spay-and-neuter-certificates and SOS will reimburse you me for the full cost of the certificate upon receiving spay/neuter documentation. You understand that the attending vet may charge additional fees that you will be responsible for. Need to get SOS enrolled

·  Vet of Your Choice. You may choose to have the surgery done at a vet of you choice. If this option is selected, SOS will reimburse up to $90/$64 for a spay/neuter, respectively, upon receiving documentation; you will be responsible for any additional fees.
