Agenda:

  1. Health Promotion Coordinator Highlight –Stephanie Quade, Chippewa SokaogonMole Lake Tribe

Congratulations to Mole Lake Chippewa Sokaogon Tribe in receiving one of 4 national Administration on Aging/Administration on Aging Tribal grants to disseminatethe Stepping On Program within the Chippewa Sokaogon Mole Lake Tribe and now continuing to expand with being awarded a second ACL/AoA grant to disseminate the Healthy Living with Diabetes program.

Mole Lake Aging Unit partnered with Mole Lake Tribal Health Department to train leaders, and recruit participants. Marketed workshops through two tribal newsletters and many tribal elders responded to invitations because they had a fear of falling. Started out with just tribal members and now have expanded to partnering with Forest County non-native residents. Participants have taken many of the concepts to heart by incorporating many of the skills in their daily life. Working with the Forest County Health department, Forest County Aging unit, and the ADRC of the Northwoods. All of the guest experts provide their presentations as an in-kind donation.

Plans for 2017 is to continue to involve at least 30 participants with Stepping On in addition to rolling out their CDSME activities.

  2. Stepping On Supplemental Leader Toolkit to Address the Needs of

Participants with Low Vision or Who Are Hard of Hearing- Shannon Myers

Shannon talked about the tool kit that was created to reach out to participants that may have low vision and are hard of hearing to make Stepping On workshops more accessible for them. Tool kit includes large print documents that are recommended for all workshops, revised registration form to help solicit accommodations needs from participants, audio descriptions of exercises assist those with low vision to follow along and information on how to obtain materials from independent living centers.

  1. Stanford CDSME Master Trainer Update: Highlights from Stanford CDSME Implementationand Fidelity Manuals: Michelle Comeau

Both Manuals were attached to email. Key updates:

  • Recommended workshops size can now be more flexible with 8-16 for rural areas and10-16 participants for urban areas.
  • Leaders should still lead workshops at least once every 12 months and Michelle will provide information regarding additional opportunities for a refreshertraining either in-person or on-line
  • Fidelity manual has updated recommendations for maintaining fidelity, and guidelines for key weekly activities
  • Sample scenarios for interview questions that can be used to recruit leaders and Memorandum of Understanding documents
  • Good practice to familiarize yourself with what Stanford recommends.

3 Healthy Living with Diabetes Master Trainers will be participating in the Stanford refresher Trainer for HLWD. Leaders to be trained in the October HLWD training will be trained in the new HLWD curriculum. WIHA will be providing information through the Health Promotion Coordinator and HLWD list serves and through the HPC webinars how refresher training will be provided to all of the existing HLWD leaders that want to move forward. This process is similar to what we did to roll out the Living Well with Chronic conditions refresher in February 2013.

  1. 2017 Leader workshop survey and 2017 training plan needs- Michelle Comeau

Survey has been available and encourage all to submit information about training needs that you anticipate. Please participate in survey as this helps us decide where training events should be located in 2017. The training schedule for 2017 will responsive so that local groups are ready to provide workshops in the most popular times that are Spring and Summer allow you to be ready for The workshop survey is also to anticipate technical assistance needs you may have.

  1. Health Care Referral and Relationships Learning Collaborative– Coming Soon!-Betsy

Betsy talked about the opportunity to participate in anticipated learning collaborative events to assist aging units that are working with health care partners in health promotion activities.

  1. Tai Chi Moving for Better Balance training opportunity October 14/15, 2016– Anne Hvizdak

TCMBB training will be held in Stevens Point at the Portage County Aging and disability Resource Center. Training is being made available from GWAAR with carryover funds. There is space for 25 participates, and the registration materials are being made available to the first 25 who had expressed interest to Jill from participants at the Healthy Aging Summit presentation this past June and attendees from the Health Promotion Coordinator’s call in April 2016. GWAAR will cover registration, leader material and food during the training. Local agencies sending participants are responsible for lodging and travel. Notice will also be sent to Aging Directors and Health Promotion Coordinators interested in being included in the waiting list. Anne Hvizdak will be assisting Carolyn Inglefield, Special Project Assistant at the GWAAR Waukesha office. If you have any questions about the training please contact Anne Hvizdak

  1. Falls Prevention Awareness Day and Month Health- Anne Hvizdak

Senator Tammi Baldwin has been invited to appear at the WAAN conference. During the week of Falls Prevention Awareness Month EPIC, an electronic health records company is rolling out the STEADI algorithm that health systems can utilize to help screen older adults for falls. EPIC and UW-Health partnered in a pilot project to develop the training. Because Senator Baldwin is very interested and supportive of activities at the CDC, an invitation was made to invite Senator Baldwin to help us celebrate Falls Prevention Awareness Day with us at the WAAN conference on September 22nd.

Kris Krasnowski talked about WIHA Falls Prevention Awareness Day and Month activities. A press conference will be held at the state capitol on Wednesday September 21. Invited guests from the DPH, CDC representatives that are working with the CDC, Representative from UW-Health that worked on the STEADI pilot, Senator Luther Olsen and WIHA representative will be presenters during the event. A press packet that will be developed will be shared with all of our health promotion partners. A Statewide Proclamation has been requested Proclamation( ** and as of 9-1-16 the statewide proclamation has been received) .

We are hoping this assists with your recruitment of falls prevention promotion and workshop activities.

See notes below – the Wisconsin Association of Senior Centers grantees conducting the Age Mastery Program have all completed their first series and planning additional workshop series this summer. A great opportunity for Age Master Program completers are great candidates for evidence based health promotion programs you are presenting. We encourage collaboration between HPC and Age Master Program Coordinators. Please contact Anne Hvizdak if you have more questions.

A resource very helpful for future planning has recently been updated. If you had a chance to hear Karen Timberlake at the Pre-summit learning Forum works at the UW Population Institute and is instrumental in creating the County Health Ranking and Roadmap

A great opportunity for you to participate in changes you see needed for the next version of the 10 year state health plan. Sign up for the WI-HIPP email updates to receive notifications about the health assessment and health improvement plan, including the final document.

Additional comments from WIHA: Michelle encourage anyone to contact her regarding technical asisstance. Betsy had suggested that WIHA will have an exhibit booth and most of the staff will be present at the Wisconsin Aging Network conference. Brenna discussed that the WIHA website is being updated and want to make sure items you use that are redily avaialble . If you have any suggestions for improvement to the website please let WIHA know.

In 2017, regional forums will be held to assist with leader development and technical assistance with marketing workshops.

Anne Hvizdak talked about upcoming changes with the Older Americans Act. There will be a training event for Health Promotion Coordinators to hear the message being shared with Aging Unit Driectors and SAMS data entry staff within the county Aging units. The changes center around the way county aging units will record their acitvity for evidence based health promotion programs and health promotion activities that are not evidence based.

Age Mastery Program (AMP)

Message from Anne Hvizdak to Health Promotion Coordinators regarding the Age Mastery Program

Hello Health Promotion Coordinators:

In recent Health Promotion Coordinators calls we talked about the Age Mastery Program being implemented in select Senior Centers around Wisconsin as a joint project between Greater Wisconsin Agency on Aging Resources Inc. (GWAAR) and Wisconsin Association of Senior Centers (WASC). The AMP projects are going well and one of the great benefits from the program is that 55% of program completers like to enroll in evidence-based health promotion programs (EBHPP) to continue learning about subjects that were introduced to in the 10 week AMP. Many of the sites have completed their 1st series of the 10 week sessions and are into the 2nd and some are on their third program. If you have not connected with the coordinator for the local Age Mastery Program yet, this would be a good time to promote the EBHPP that are planned for your area.

The Aging Mastery Program® (AMP) helps baby boomers and older adults take key steps to improve their well-being, add stability to their lives, and strengthen their ties to communities. The states that have implemented the program have had great success! This 10-week program is the perfect gate-way for all the outstanding Evidence Based Programs supported by WIHA. We encourage you to work in collaboration with your local senior center, aging office/ADRC, healthcare providers and others in your community that support healthy aging.

Good Afternoon!

First, congratulations on successfully completing your first round of classes!! We have received excellent feedback and wanted to thank you for your outstanding work. We realize there are a few of you who are getting started late, but we are confident that you will be up to speed soon and get rolling with the classes.

Collaborating with ADRCs and Aging Units.Just a reminder, if you are not currently working with your local ADRC and/or Aging Unit to promote and expand AMP please contact your local office. We have gotten feedback that they very much want to be involved in helping to grow AMP in WI. They have newsletters, websites, Senior Dining Centers and other forms of marketing and outreach that can help you reach a broader audience. They also have ties to many other agencies so it is a win-win. My sincere apologies for not stressing this sooner, it’s a learning process. Thank you in advance. Please let me know if you would like me to facilitate an introduction or meeting with your local Aging Unit and/or ADRC and I would be happy to facilitate the meeting. I will also be reaching out to the following agencies to let them know about the success of the program to date and remind them that you are the working under this grant to bring AMP to WI and that the goal is to grow and sustain the program so the more collaboration the better.
