TVI CANDIDATE GRANT PLANNER
Introduction
This CandidateGrant Planner is a useful tool for helping your PEA to complete the Grants Management Enterprise (GME) grant application. Use the Request for Proposals (RFP) General Instructions Summary with this Grant Planner to plan and develop the candidate’s portion of the grant. Savethe completed Grant Plannerin Microsoft Word on a laptop or personal computer and forward via email to the local project coordinator. The information you provide in this document will be imported into the Arizona Department of Education, Grants Management Enterprise (ADE/GME), electronic application template. An electronic grant application that is found to be erroneous or incomplete will be rejected for revisions. You may be called on by the local project coordinator to make corrections, so please follow all instructions provided to you in the RFP and Grant Planner.
Grant Submission Deadline
The local project coordinator will set a date for the return of your completed Grant Planner. Please follow the instructions provided by that person.
Drafting Narrative Responses
The Grant Planner’s text cells will expand to accommodate the text that is typed into them; however, we recommend that you limit text to no more than 200 words for the one required narrative. The narrative is flagged by “[Narrative].”
Please conduct a character count on the narrative and a spell check on the information in all text cells prior to copying and pasting text into the GME. To conduct a spell check, select the text; click on the “Review” tab; click on “ABC Spelling & Grammar.” To count the number of characters, select the text; click on the “Review”tab; click on “Word Count.”

Additional Submission Requirement
Supplemental Information Packet (SIP)
To complete the application process and receive approval for the tuition scholarship, a Candidate must forward copies of the following supplemental documents to the ADE/ESS designee unless otherwise instructed by the local project coordinator. The local project coordinator may choose to coordinate all grant submissions and ask that you provide this information to him or her instead of sending it directly to the ADE/ESS designee.
The ADE/ESS designee’s name and address are located at the top of the SIP Cover Sheet. The SIP must include the following information. Please follow the instructions to the letter. If you have any questions, feel free to contact Celia Kujawski at 602-432-3213 or .
Letter of Interest
The UA/DPS requires a letter of interest from the Candidate that describes his or her background and interest in the VI program. The original, signed letter of interest should be mailed to UA/DPS to fulfill university program entrance requirements. A copy of the letter of interest must be sent to ADE/ESS as part of the grant application.
Letters of Reference
The UA/DPS requires each participating Candidate to submit three letters of reference from individuals who are knowledgeable of that person’s professional work and can address performance in the areas of leadership, scholarship, and written communication (i.e., college professors and administrative superiors). Original, signedletters should be forwarded to UA/DPS to fulfill university program entrance requirements. A copyof each must be sent to ADE/ESS as part of the grant application.
Tuition Agreement
The Tuition Agreement defines the partnership responsibilities of the Candidate as a student in the program, employing PEA, and ADE/ESS. It must be signed by the superintendent of schools or director of charter schools, local special education director, and Candidate. The original,signed Tuition Agreement must be sent with the copies of the letters of reference and letter of interest to ADE/ESS.

note:ADE/ESS recommends that the SIP be mailed to the ADE/ESS designee with proof of postmark (e.g., documentation of delivery/receipt). We are not responsible for:
1.A SIP that gets lost and does not have proof of delivery/receipt.
2.A SIP that is incorrectly addressed and does not get rerouted to the designee.
3.Maintaining a copy of the SIP for your records. It is good business practice to copy all documentation for your files prior to mailing it to the ADE/ESS designee.
