Documents sent to the Collaborator

The Collaborator will receive:

·  Italian contract to be signed

·  English contract to be signed

·  Three copies of the “Host Agreement”

Documents sent by the collaborator

The collaborator must send back:

·  Italian contract signed

·  English contract signed

·  Three copies of the “Host Agreement”

·  Photocopy of degree

·  Original copy of the Curriculum vitae signed and with the date when the degree was achieved

·  Two readable photocopies of the passport

The Scientist-in-Charge/The Collaborator will communicate the address in Italy where he/she will reside.

Once in Italy, it is necessary to get copy of valid proper housing certification or receipt of the application released by Comune or Asl and a declaration of hospitality written by the host on plain paper.

Applying for entrance visa

Once obtained the authorization, the original document is sent to the Collaborator who will go the Italian Consulate in his/her own country to apply for the entrance visa for self-employed work.

Once obtained the visa, the Collaborator will enter Italy and will go to Prefettura on date established in agreement with University. Here he /she will bring:

- copy of valid proper housing certification or receipt of application released by Comune or Asl;

- declaration of hospitality, written by the host on plain paper;

- photocopy of the ID document of the host;

- photocopy of the whole passport of the foreign collaborator, inclusive of the entrance visa;

- tax code if already owned by the collaborator;

Prefettura will release copy of stay contract (Mod209) and envelope for sending documents at the Post Office.

The Collaborator will send the following documents by post:

1.  Modello 209 in original signed and printed with 14,62 Euro stamp;

2.  photocopy of the whole passport inclusive of blank pages;

3.  photocopy of the tax code;

4.  payment form (PSE) (Euro 27,50).

Sending the kit costs 30 Euro. Copy of the receipt of such registered letter must be sent to Divisione Stipendi (fax 02 50313255).

A date for taking finger prints at Questura will be set through the Post Office.

While waiting for permission of stay, it is allowed to start activity.

Once obtained permission of stay the collaborator will send a copy to Divisione Stipendi (fax 02 50313255)

Registering in Public Health Insurance

The collaborator will go to ASL administrative office, present in every ASL, with the receipt of the registered letter above mentioned, a copy of the contract with University, tax code, certification or self-certification of residence.
