National Honor Society Community Service Hours
Name (Please print first and last name) ______
Please list the community service hrs that you have done for the second semester of the 2013-2014 school year that are not scheduled activities of NHS. (I have the Polar plunge, Special Olympics Basketball, P/T conferences, Puddle Jump, the Eau Claire Marathon, the Homeless event, and Pump for a Cause, so you do not need to list these.)
Tutoring Hours (Tutoring is considered an NHS sponsored activity and counts as NHS hours (note: Read and Lead at Flynn is a Key Club event only and can count as other hours outside of NHS):
NHS activity/tutoring / Hours / Signature of supervisor (adult only)Other Community Service Hours outside of NHS:
Type of Community Service/location / Hours / Signature of supervisorReminder: Each semester, NHS members are required to do 10 hrs of community service. One half of the hours are supposed to be with NHS, and the other half may be served with any club, church, or community non profit organization. NHS has had multiple opportunities this semester to earn these hrs by working parent teacher conferences, participating in Polar Plunge, tutoring at South, Meadowview, Putnam Heights, (tutoring is counted as an NHS activity no matter where you do it-(as long as an adult signs off) ,This adult has to be your supervisor-NOT YOUR PARENT!!) helping with the Amazing Eau Claire Cleanup, and volunteering at other NHS sponsored events such as the Special Olympics Pump for a Cause Day.
As the new NHS members joined at the beginning of 4th quarter, they will only have to do 5 service hrs.. Therefore, 2.5 hrs must be with NHS and 2.5 hrs. of other service (with the NHS or anywhere else.)
Please turn this in this form to Mrs. Lange at the May 21(underclassmen) or May 7(seniors) meeting in the library at 7:10 am.
Thank you!!
