D1. For FSP/BCM/EEP/RMC/OTHERS

Type 2 Type 1b Type 5 OTHERS

  1. ISO/IEC Type 2 Certification Scheme consists of type testing of a sample and market surveillance. Market surveillance is conducted and samples of the product from the market are assessed for ongoing conformity.
  1. ISO/IEC Type 1b Certification Scheme consists of type testing of a sample of a production and subsequent batch inspection. Regulated fire safety products certified under ISO/IEC Type 1b Certification Scheme are issued with product labels which are also displayed on the products.
  1. ISO/IEC Type 5 Certification Scheme consists of testing and assessment of the quality system involved. Surveillance of the quality system is conducted and samples of the product from either the market or at the point of production, or both, are assessed for ongoing conformity. Regulated fire safety products certified under ISO/IEC Type 5 Certification Scheme (Discrete System) are issued with product lables which are also displayed on the products.

Refer to Scope of Certification for RMCattached at the back and complete the details of concrete family and the individual concrete products.

F. Supporting Documents

Please ensure that the following documents are submitted together with the application form where appropriate:

Original or copy of test / assessment report(s) bearing recognized accreditation logo with certification number.

(Test report validity period shall be 5 years for products listed under PLS with the issue of product labels which can be displayed on the products.

Test report validity period shall be 10 years for product listed under PLS with the issue of declaration of conformity, DOC.)

Product specification (eg. Component list, material list, drawing and product sample)

Original or copy of latest information from the Accounting and Corporate Regulatory Authority (ACRA)

Original or certified true copy of authorization letter from manufacturer if applicant is not the manufacturer confirming that applicant is authorized to deal in the product.

Original or certified true copy of the authorization letter from the owner of test report (if the applicant is not the owner of test report) confirming that the applicant is authorized to use the test report for application.

Copy of Certificate(s)to declared certification (e.g. ISO 9001, 14001, 18001, etc)

Risk Assessment Form(s), Safety Work Process, or any Safety-related documentation for critical/high-risk activities that is within the scope of assessment/inspection.

Part 2. APPLICATION FEE

1. The application fee is only valid for 1 year from acceptance of this form. If an applicant is not able to obtain certification within 1 year from the date of application, a new application form and fee have to be submitted.

2. Refer to Fee Schedule for the corresponding amount to be paid.

3. Cheque(s) shall be crossed and made payable to SETSCO Services Pte Ltd.

Part 3. CONTRACT AGREEMENT

Refer to Terms and Conditions for Certification attached at the back.

Part 4. DECLARATION

  1. The organization named above applies for certification for the scope(s) set out in this application. I declare that the information on this form and any information given in support of this application are correct and accurate to the best of my knowledge.

Moreover, I acknowledge that the informationdeclared herein shall only be valid within one(1) month from CB’s acceptance date. If for any reason, an assessment has not been confirmed within the said period, it will be my responsibility to update the CB of my organization’s information by resending a new application form.

  1. I have read the Terms and Conditions for Certification, criteria and applicable normative documents, and undertake that the organization will comply with these requirements if this application is accepted.
  2. I undertake that the organization shall pay all fees due to SETSCO, whether or not certification is granted.
  3. I declare that our company() does not engage any consultancy from SETSCO on the product intended for this application.
