CSE Criterion # 13 - Progress Reports and Content

Basis for Findings:
Review of student records and staff interviews indicated that, at times, progress reports were written on goals from IEPs that had not yet been signed and accepted by the parent/guardian. Additionally, the collaborative does not have a process in place to monitor progress reporting.
Department Order of Corrective Action:
Develop procedures for staff to follow to ensure that progress reports are written on goals in IEPs that have been consented-to by the parent. Train appropriate staff on these procedures.
Develop an internal monitoring system to ensure that progress reports are written on goals from IEPs in which parents have provided signed consent. The monitoring system should identify a person(s) who will be responsible for reviewing progress reports and maintaining continued compliance.
Conduct an internal review of progress reports, written subsequent to implementation of all corrective actions,for evidence that all progress reports are written on goals from consented-toIEPs.
*Please note that when conducting internal monitoring the collaborative must maintain the following documentation and make it available to the Department upon request:
a) List of student names and grade levels for the records reviewed; b) Date of the review; c) Name of person(s) who conducted the review, with their role(s) and signature(s).
Required Elements of Progress Reports:
Submit the procedures andevidence of staff training, including signed attendance sheets and agendas by November 18, 2016.
Provide a description of the internal monitoring system, including the name(s) of the responsible personnel by November 18, 2016.
Submit the results of the review of student records and include the following:
1. The number of records reviewed;
2. The number of records in compliance;
3. For any records not in compliance, determine the root cause; and
4. The specific corrective actions taken to remedy the non-compliance.
Please submit the above information by February 22, 2017.

Review of student records and staff interviews indicated that at the beginning of the school year the collaborative has a current IEP in effect for every student.
CSE Criterion # 29 - Communications are in English and primary language of the home

Basis for Findings:
At the time of the review, there were no students enrolled in the collaborative whose primary language of the home was other than English. Document review and staff interviews indicated that the collaborative has procedures in place to provide communications to parents in both English and the primary language of the home, if the primary language is other than English.
Specifically, the intake form is used to identify the language of the home. If the primary language of the home is not English, the collaborative will have documents translated, as needed. The collaborative has a contract with a translation agency to translate documents.
CSE Criterion # 45 - Procedures for suspension up to 10 days and after 10 days: General requirements

Review of student records, staff interviews and document review indicated that the collaborativeuses a suspension log to document both in-school and out-of-school suspensions.Program directors review the log monthly to monitor suspensions andidentify any students who are approaching ten days of suspension.
