Release Form

Terms and Conditions:

  1. By entering this competition, I agree to be bound by these terms and conditions.
  2. I confirm that each individual whose image is featured in my entry has given consent for the use of his/her image in connection with this competition and in accordance with these terms and conditions. If my entry features an individual aged less than 18 years, I confirm that the parent or guardian of that individual has given the relevant consent. I agree that the organiser may request satisfactory evidence of such consent and that the organiser’s decision is final.
  3. There is no entry fee and no purchase necessary to enter this competition.
  4. Only one entry will be accepted per person. Multiple entries from the same person will be disqualified.
  5. The organisers’ decision in respect of all matters to do with the competition will be final and no correspondence will be entered into.
  6. The organisers reserve the right to cancel or amend the competition and these terms and conditions without notice in the event of any unexpected event or anticipated breach of any applicable law or regulation or any other occurrence outside of the organizer’s control. Any changes to the competition will be notified to you as soon as reasonably practicable possible by the organizers.
  7. I understand that my personal information will not be shared with third party organisations and will be treated in accordance with the Data Protection Act 1998.
  8. I understand that my name will be published on the London School of Hygiene and Tropical Medicine, Mental Health Innovation Network and CGMH websites if I am selected as one of the three winners.
  9. I agree to provide the London School of Hygiene & Tropical Medicine with a licence to the copyright in my entry, enabling it to publish or use or reproduce your entry for the purpose of the photography exhibition, future public engagement events, website and research activities, publicity/advertising or any other lawful purpose in conjunction with your name, or in reproductions thereof in colour or otherwise.
  10. I agree that the licence means that my entry can be used anywhere in the world, that it cannot be revoked, and that it will last in perpetuity.
  11. I agree to waive all claims for any compensation for such use and agree to release the London School of Hygiene & Tropical Medicine from all liabilities or claims that I could assert in connection with the above described uses.
  12. I hereby waive any right to inspect and/or approve the finished product or the use to which it may be applied.
