EQUIPMENT LOAN REQUEST FORM
A valid BruinCard will be required to check out any equipment from the Community Programs Office (CPO). Users without a valid BruinCard may not check out any equipment. Equipment includes but is not limited to: projectors, laptops, speakers, digital cameras, and video equipment. Privileges may be revoked or suspended based on violations of policies at the discretion of the CPO Technology Manager.
How to Check Out
1. Submit an email to at least 2 business days in advance
2. Email Subject Line: (Project Name) Request for (indicate which type of equipment)
3. Email Body:
a. (Project Name) Request for (indicate which type of equipment)
b. Purpose: Meeting / event etc.
c. Pick-Up Date & Time
d. Return Date & Time
e. Contact Phone Number

* For equipment usage before 10a, please make sure you pick them up the business day before by 6p
**In the academic year, our office is not open for any equipment returns or checkouts on Fridays. If you would like to use the equipment on a Friday, please make sure you pick-up the item on Thursday by 6p

4. Confirmation Email: After you have submit an email request, you will receive a confirmation from the Technology Manager within one business day, confirming or denying your request
5. If your request has been approved, please approach the SAC 100C on the approved date and time and present a valid BruinCard (no other form of identification can be accepted)
6. Equipment is not reserved until you receive a confirmation email
7. Borrowers must leave their BruinCard with the CPO Technology Manager or Technology Team Member until time of return
8. Borrowers must not check out equipment for use by any other student or individual. Doing so will result in permanent revocation of borrowing privileges.
9. All equipment is not to be taken off UCLA’s campus unless otherwise previously approved. The student/project is responsible for any damage to the equipment. The student/project will be charged accordingly based on the cost incurred by the CPO to repair the damage or to replace the equipment.
Returning Equipment
· All equipment returns must be at SAC 100C. Items dropped off at locations other than the original borrowing locations are not considered returned and will accrue late fines until they are retrieved by the borrower and returned to the correct location.
· In the academic year, we donotallow any equipment returns on Fridays.
· Please return all equipment on time.
· An inventory will be made of all these items. If something is missing or damages were incurred, it will be noted and you may be fined. Also, if you noticed a problem with the equipment, such as an application not working or turning on, please notify the CPO Tech Manager immediately.
· BruinCard will be returned to borrower upon equipment return if there are no issues.
Fees & Fines
Lost/Stolen/Damaged equipment
· You are responsible for the full replacement cost of up to $500 if equipment is lost or stolen or damaged. The charge will be made to your Bruin Account.
· Problems with, damage to, loss or theft of the hardware loaned to you must be reported immediately. Failure to do so will result in repair and/or replacement fees that you are responsible for. Any damage to, loss or theft of loaned equipment during the loan period is the sole responsibility of the borrower as well as subsequent repair or replacement costs up to the original value of the item(s).
Late Returns
· Returning the equipment after it is due will result in a late fee of $2.50 per hour, or any fraction thereof, up to $25.00 per day. The charge will be made to your Bruin Account.
· Unpaid fees or fines will prevent you from registering for classes and could delay or prevent you from graduating.
Failure to Return
· If you have not returned the equipment system before the CPO closes, you must keep the equipment in your possession and return it first thing the following morning to the SAC 100C. You will be charged a $25.00 late return fee in addition to any hourly fine incurred. Under no circumstances should the equipment be left unattended in the SAC after closing.
When submitting an email to request equipment, you are indicating that you have read and agree to the terms listed above.
